Aan het begin van de pandemie maakten clinici gebruik van computertomografie (CT) om een snelle diagnose te stellen en voor de risicostratificatie van pati\u00ebnten met een verdenking op COVID-19. De multicenterstudie in hoofdstuk 2 toonde aan dat het CO-RADS (COVID-19 Reporting and Data System) een uitstekend hulpmiddel is om onderscheid te maken tussen positieve en negatieve COVID-19-pati\u00ebnten op de SEH. Bovendien had de mate van longbetrokkenheid bij presentatie op de SEH \u2013 gekwantificeerd door de CT-severity score (CTSS) \u2013 een significante positieve associatie met ziekenhuisopname, opname op de intensive care (IC) en 30-dagen mortaliteit. In hoofdstuk 3 lieten we zien dat procalcitonine (PCT) kan helpen bij het uitsluiten van een bacteri\u00eble (co-)infectie bij pati\u00ebnten met respiratoire symptomen op de SEH. We vonden dat de PCT-afkapwaarde voor een bacteri\u00eble infectie waarschijnlijk verhoogd kan worden naar 0,5 \u03bcg\/L bij pati\u00ebnten met een bewezen virale infectie. In hoofdstuk 4 ontwikkelden en valideerden we extern een voorspellingsmodel voor een slechte uitkomst (IC-opname of 30-dagen mortaliteit): de COVERED-risicoscore. Deze toonde goede discriminatie en kalibratie in zowel het Nederlandse cohort als een Italiaans validatiecohort.\n\nIn deel 2 onderzochten we het gebruik van point-of-care echografie (POCUS) voor diagnose en prognose in de acute zorg. De diagnostische eigenschappen van long-echografie (LUS) zijn beter dan die van een r\u00f6ntgenfoto van de borstkas en vrijwel gelijkwaardig aan een CT-scan voor de diagnose van diverse acute longaandoeningen. Hoofdstuk 6 toonde aan dat een 12-zone LUS een vergelijkbaar uitstekende negatieve likelihood ratio (NLR) had als CT voor de diagnose van COVID-19-pneumonie. In hoofdstuk 7 vergeleken we verschillende LUS-protocollen en vonden we dat het 6-punts protocol een uitstekend screeningsinstrument is. De bevindingen in hoofdstuk 8 laten zien dat de semi-kwantitatieve LUSS bij presentatie op de SEH geassocieerd is met een slechte uitkomst, de opnameduur en de ernst van de ziekte. In hoofdstuk 9 screenden we pati\u00ebnten op de verpleegafdeling op asymptomatische diepveneuze trombose (DVT), waarbij een lage prevalentie (4%) werd gevonden, wat suggereert dat systematische screening in deze setting niet zinvol is.\n\nDeel 3 richtte zich op POCUS voor het monitoren van COVID-19 op de IC. Hoofdstuk 10 toonde een uitstekende overeenstemming aan tussen 6-zone en 12-zone LUS-scores in zowel rug- als buikligging, wat aangeeft dat het tijdrovende 12-zone protocol niet noodzakelijk is. In hoofdstuk 12 vonden we dat de LUSS binnen 24 uur na intubatie geassocieerd was met succesvolle ontwenning van de beademing. Hoofdstuk 13 stelde vast dat seri\u00eble LUSS in staat is om werkelijke veranderingen in longbetrokkenheid te detecteren en dat een stijging van de LUSS na 2 weken geassocieerd is met mortaliteit. In hoofdstuk 14 onderzochten we de nauwkeurigheid van multi-orgaan POCUS voor het detecteren van longembolie\u00ebn op de IC. Een combinatie van echo van de diepe aderen en het hart bleek nuttig bij het uitsluiten van longembolie\u00ebn. In hoofdstuk 15 onderzochten we de evolutie van de dikte van de ademhalingsspieren, waarbij we zagen dat het gebruik van spierverslappers geassocieerd was met spierverlies, terwijl positieve eind-expiratoire druk (PEEP) geassocieerd was met een toename van de diafragmadikte.","summary":"Part 1 of this thesis focused on how conventional diagnostic tests could aid in the diagnosis and prognosis of (suspected) COVID-19, as well as its complications in patients presenting to the emergency department (ED). At the start of the pandemic clinicians turned to computed tomography (CT) to aid rapid diagnosis, as well as risk stratification of suspected COVID-19 patients. The multicenter study in Chapter 2 showed that the CO-RADS (COVID-19 Reporting and Data System) is an excellent tool in differentiating between positive and negative COVID-19 patients at the ED. Furthermore, the degree of pulmonary involvement at ED presentation \u2013 quantified by the CT severity score (CTSS) \u2013 had a significant positive association with hospital admission, intensive care unit (ICU) admission and 30-day mortality. In Chapter 3, we showed that procalcitonin (PCT) may aid in excluding a bacterial (co)infection in patients with respiratory symptoms presenting to the ED. We found that the PCT cutoff for a bacterial co(infection) can probably be raised to 0.5 \u03bcg\/L in patients with a proven viral infection. In Chapter 4, we developed and externally validated a prognostication model to predict poor outcome (ICU admission or 30-day mortality) in patients with suspected COVID-19: the COVERED risk score. It showed good discrimination and calibration in both the Dutch derivation cohort and an Italian validation cohort.\n\nIn Part 2, we explored the use of point-of-care ultrasound (POCUS) for diagnosis and prognosis in the acute care setting. LUS (Lung Ultrasound) diagnostic properties are better than chest X-ray (CXR) and almost equivalent to chest CT in diagnosing various acute respiratory pathologies including pneumonia and ARDS. The results of Chapter 6 showed that 12-zone LUS had a comparable excellent negative likelihood ratio (NLR) to CT in diagnosing COVID-19 pneumonia. In Chapter 7, we compared different LUS protocols and found the 6-point protocol to be an excellent screening tool due to its high NLR. The findings presented in Chapter 8 show that the semi-quantitative lung ultrasound score (LUSS) at initial ED presentation is associated with poor outcome, admission duration, and disease severity. In Chapter 9, we screened COVID-19 patients admitted to the medical ward for asymptomatic proximal deep venous thrombosis (DVT) with extended compression ultrasound (ECUS), finding a low prevalence (4%), suggesting that systematic screening for asymptomatic DVTs in this setting is not helpful.\n\nPart 3 addressed the use of POCUS in monitoring the COVID-19 disease course in the ICU. Chapter 10 showed excellent agreement between 6-zone and 12-zone LUS scores in both supine and prone positions, indicating no argument for using the more time-consuming 12-zone protocol. In Chapter 12, we found that the LUSS within 24 hours of intubation was associated with successful liberation from mechanical ventilation. Chapter 13 established that serial LUSS is capable of detecting true changes in pulmonary involvement and that a rise in LUSS after 2 weeks is significantly associated with mortality. In Chapter 14, we explored the diagnostic accuracy of multi-organ POCUS for detecting pulmonary embolism (PE) in critically ill patients. We found that a combination of deep vein and cardiac POCUS \u2013 rather than any single modality alone \u2013 can be of aid in ruling out PE.
