Summary

The overarching aim of this thesis was to contribute to more robust, patient-centered, and cost-effective care for individuals who suffer from FI. This was done by finding weaknesses in the current management and research field of FI and aiming to close these gaps.

The work began with the development of a comprehensive international guideline for the diagnosis and management of FI (chapter 2). This guideline, created in collaboration with patients and international experts produced 45 evidence-based recommendations spanning diagnostic strategies, conservative and minimally invasive treatments, and surgical interventions. The guidelines demonstrate which diagnostics tools and treatment options should or should not be used in patients with FI. The main take-away from the resulting diagnosis and treatment pathway is that order of treatment and approach should be decided through shared decision making, by the patient and healthcare professional. Patients’ preferences as well as lifestyle factors and fitness level for possible procedure should be considered.

Furthermore, during the development of this guideline, a major limitation in the FI research field became apparent: there was considerable heterogeneity in how outcomes were defined, measured, and reported across FI studies. Clinical trials used different instruments or poorly defined endpoints, making it difficult to compare results or perform meta-analyses. Moreover, most research focused narrowly on physiological or symptom-based outcomes, such as incontinence frequency or stool consistency, while psychosocial measures, besides ‘quality of life’ were rarely included. This inconsistency not only limited the strength of the guideline recommendations but also highlighted a fundamental disconnect between outcomes used in research and outcomes patients may prioritise.

To address this gap, subsequent studies within this thesis focused on understanding which outcomes truly matter to people living with FI and on developing a standardised approach to outcome reporting. Chapter 3 describes the process of the development of a Core Outcome Set (COS) for FI. The first step of this COS involved a qualitative exploration of patients’ lived experiences (chapter 4). Interviews revealed the profound impact of FI on daily life: constant fear of accidents, loss of spontaneity, and avoidance of social situations were recurring themes. Patients described the emotional burden of shame, isolation, and reduced self-esteem, as well as the coping strategies they adopted to maintain dignity and control. Importantly, participants expressed that while symptom improvement was valued, their greatest priorities were regaining confidence, autonomy, and the ability to participate in daily and social activities without fear. These insights underscored a mismatch between the outcomes emphasised in clinical trials and those most meaningful to patients.

Building on these findings, a systematic review of interventional FI studies was conducted to quantify the extent of heterogeneity in outcome reporting (chapter 5). The review identified more than 250 distinct outcomes, measured using over 100 different tools, many of which lacked validation. This variability severely limits comparability between studies and hinders evidence synthesis. Most studies focused on physical symptoms, while few addressed psychosocial wellbeing, coping, or quality of life. The review reinforced the urgent need for a unified framework to guide consistent, relevant, and patient-centred outcome measurement in FI research.

The outcomes identified as important during the patient interviews and the outcomes identified through the systematic review were combined and put forward to an international group of patients, healthcare professionals and researchers to rank and refine outcomes. The final step was a consensus meeting, with the relevant stakeholders, to agree on the key outcomes that should be measured and reported in all future FI studies. Importantly, patients were given power of veto with regards to the final selection of the items for the COS. The resulting list of outcomes captures both domains of quality of life (QoL) and clinical outcomes. The QoL outcomes included in the COS are: general QoL, influence on daily activities, social functioning, treatment satisfaction, enjoyment in life, embarrassment and peace of mind. The clinical outcomes included in the COS are: severity of FI, number of FI episodes, urgency, stool consistency, adverse events, and adherence to therapy. This represents the first internationally agreed standard for outcome reporting in FI research.

In parallel, this thesis also examined the economic and societal burden of FI and evaluated a novel minimally invasive intervention. The cost-of-illness study (chapter 7) demonstrated that patients who suffer from FI impose a substantial financial burden to society. It was estimated that the average societal quarterly costs of a person with FI was €2,400, and this included costs for healthcare, productivity loss, and informal caregiving. These findings position FI as a condition with a socioeconomic impact comparable to other chronic diseases such as diabetes or inflammatory bowel disease, yet it remains markedly underrepresented in health policy and research funding.

This thesis also sought to specifically examine the therapeutic efficacy of a novel therapy for FI, aimed to fill the gap between first-line conservative treatments (such as loperamide and pelvic floor therapy) and more invasive therapies (such as sacral nerve modulation or ostomy). The CONFIDEnCE trial (chapter 8), a multicentre randomised controlled study, assessed a novel anal insert device compared with care as usual. Participants in both groups were requested to continue their usual FI treatment to stimulate the clinical setting in which the insert would be used. Participants in the intervention had to additionally use the anal insert. While no statistically significant difference was observed in the primary outcome of ‘FI severity’, participants using the device reported significant improvements in other outcomes, including a reduction of approximately three FI episodes per week, as well as better coping, and a lower depression score.

Alongside the clinical evaluation, a cost-effectiveness analysis (chapter 9) was conducted to assess the cost-effectiveness of the anal insert compared to care as usual. From a societal perspective, the intervention resulted in similar costs to the intervention group (adjusted mean difference: €3; 95% CI: -€940 to €933) and a negligible quality adjusted life year (QALY) difference (-0.004; 95% CI: -0.016 to 0.008), resulting in an ICER of €962 per QALY and was deemed not cost-effective. However, the anal insert showed potential cost-effectiveness among patients who adhered to treatment and did not discontinue the trial due to lack of efficacy. Hence, although not universally effective, the analysis demonstrates that the insert could be cost-effective in real world clinical practice, where patients typically undergo an initial trial period: those who experience benefit will continue using the device, while those who did not respond discontinue treatment.

In conclusion, this thesis highlights the need for patient-centered approaches in both clinical care and research in FI. The final part of the thesis formulates futures perspectives and attempts to provide a guide to properly address the remaining knowledge gaps.
["GET
