The immune system is a powerful machinery designed to protect us from external dangers such as virus and bacteria and to keep the order internally by eliminating rogue cells such as cancer. Unfortunately, during the past few decades, cases are on the rise where the immune system itself represents a threat. These range from autoimmunity to allergy, which arise when the balance between immune activation and tolerance is lost. Equilibrium in the immune system is kept by a series of breaks and checkpoints.

One of these breaks, which is gaining more and more attention, is represented by the sialic acid/Siglec axis. Sialic acid is a monosaccharide which caps mammalian glycans. Given its position as outermost glycan, it is used as self-associated molecular pattern, to signal “self” to the immune system. Accordingly, cells of the immune system, specifically belonging to the myeloid compartment, are equipped with receptors called Siglecs to sense sialylated moieties and suppress the response. Cancer cells exploit this axis to evade immune surveillance, by increasing the sialylation pathway and the concentration of sialylated glycans on their surface to be seen as “self”.

The research conducted in this thesis was guided by the idea that the natural immunosuppressive properties of the sialic acid / Siglec axis could be harnessed to reprogram the immune response to FVIII in hemophilia A and Der p 2 in house dust mite allergy towards tolerance. As reviewed in Chapter 1, FVIII is a coagulation factor that is used in replacement therapies for hemophilia A patients, whom presents with a genetic mutation in the FVIII locus which hampers the coagulation cascade. In up to 30% severe cases the patients develop anti-FVIII neutralizing antibodies which makes the therapy ineffective. On the other hand, Der p 2 is a protein produced by house dust mite. It is an unharmful molecule for humans, but some individuals develop an immune response towards it guided by IgEs which culminates in exacerbation of asthma, sneezing, coughing and itch. We hypothesized that by coupling FVIII and Der p 2 to a sialylated glycan we could engage Siglec-9 on dendritic cells and thus initiate a tolerogenic program.

First, in Chapter 2 we investigated how to study this system by developing a reporter cell line for one of the Sigecs. By engineering a Jurkat cell line with a NFAT-Luc plasmid and Siglec-7-ITAM chimera, we revealed by ELISA and luminescence that one of the most renowned Siglec-7 ligand, sialylated CD43, might not be capable of inducing signalling. We propose to use this reporter to improve the screening of targets of Siglec-7, to potentially accelerate the discovery of novel points of intervention in the treatment of cancer.

Next, we investigated how to translate this system into a pharmaceutical formulation of FVIII. In Chapter 3, we outlined a strategy to select immunodominant peptides from FVIII and chemically conjugate them to a sialic acid-terminating tetrasaccharide. We tested Siglecs binding by ELISA and further demonstrated that IL-10 secretion induced by sialylated FVIII peptides in monocyte-derived dendritic cells was Siglec-9 dependent by making use of knock-outs. In Chapter 4 we moved onto testing these sialylated FVIIII peptides in a FVIII-KO model, showing that compared with the control group treated with not-sialylated peptides there was an increase in Tregs in the spleens and lower percentages of IFNγ- and TNFα-secreting Teff.

Chapter 5 extends the investigation of sialic acid/Siglec interactions to the context of allergy. We conjugated the same sialylated glycan employed in chapters 3 and 4 to Der p 2, and analyzed its effects on peripheral blood mononuclear cells (PBMCs) from non-allergic volunteers. Our data showed that sialylated Der p 2 both suppressed the production of Th2 cells cytokines (IL-4, IL-5, IL-13), and the activation of CD4+ T cells. Moreover, higher frequencies of two subsets of lymphocyte activation gene 3 (LAG-3+) regulatory T cells were observed in PBMCs cultured with sialylated Der p 2.

In conclusion, as discussed in Chapter 6, within this thesis we investigated how directly sialylating antigens and targeting ITIM-carrying Siglecs on myeloid cells can bias the immune system towards tolerance in two different clinical contexts, inhibitors in HA and house dust mite allergy. We also highlighted how the route of delivery and the immune niche critically determine whether sialic acids drive suppression or activation, uncovering what I called the Janus-like role of sialic acid in immune regulation.
["GET
