The introduction of electronic health records (EHRs) has made routine care data available on a large scale. Most clinically relevant information, however, is not stored in structured fields but written as free text in clinical notes. This text contains local jargon, abbreviations and inconsistent terminology, which makes it inaccessible to conventional database queries. For research purposes, these data have therefore mainly been collected through manual chart review (MR), which is accurate but too slow to apply to large cohorts. Text mining converts free text into structured variables and offers a scalable alternative, provided that its accuracy and transferability are established.

The need for such data is evident in oncology. Immune checkpoint inhibitors (ICIs) have changed the treatment of non-small cell lung cancer (NSCLC), but the patients enrolled in the registration trials differ from those treated in routine care, who are older and have more comorbidities. This has consequences in three domains. The first is safety. Trials are limited in size and duration, so rare adverse events and toxicity in patients with comorbidity or reduced physiological reserve remain poorly characterized. The second is effectiveness, since survival expectations are derived from trial outcomes that may not apply to the treated population. The third is cost. Reimbursement rests on cost-effectiveness models built on the trial population, and whether these hold in practice is unknown.

Postmarketing drug safety monitoring relies largely on spontaneous reporting systems, in which underreporting delays the confirmation of safety signals. Unreported associations between drugs and adverse drug reactions are described in the EHR, and proactive searching could conform safety signal sooner.

The aim of this thesis was to validate an EHR text-mining tool, CTcue, and to apply it to strengthen pharmacovigilance and to evaluate the effectiveness, safety and costs of oncological treatment in routine care.

Section I evaluates how accurately CTcue extracts routine care data, and whether extraction algorithms function outside the hospital in which they were developed. In Chapter 2, CTcue was compared with MR in patients who underwent resection for early-stage NSCLC. Extraction was accurate for consistently documented variables such as tumor histology, surgery type and vital status, and poor for variables recorded in varying terminology, such as performance status and pathological nodal stage. Upon inclusion, each method identified patients that the other had missed, so neither yielded a complete cohort on its own. Survival outcomes were nonetheless highly comparable, and data collection was considerably faster. Extraction performance thus depends on documentation quality rather than purely on the tool, and manual verification remains necessary for poorly standardized variables.

Disease progression is not recorded in a structured field and has to be derived from free text. In Chapter 3, a collaboration study, an algorithm developed in two hospitals was applied in two others, one of which used a different EHR system. Performance remained high after transfer, and the progression-free survival (PFS) curves closely resembled those obtained manually. Complex free-text endpoints can therefore be captured reliably and shared between hospitals, provided that retrieved fragments are verified manually in context.

Section II concerns drug safety, both in the confirmation of new signals and in the toxicity of ICIs in older patients. Signals require sufficient cases before a causal association can be established, and voluntary reporting supplies these slowly. In collaboration with the Netherlands Pharmacovigilance Centre Lareb, targeted searches were performed in Chapter 4 for two established adverse drug reactions (ADRs) and two suspected associations. Additional cases were found in all four instances, and most had been documented in the EHR before the association was formally established. Searches on diagnostic terms in free text were most informative, whereas symptoms and laboratory results yielded largely non-specific results. Cases that support safety assessment are therefore present in clinical records well before they reach the reporting system, and can be retrieved once a hypothesis exists.

ICIs act by activating the immune system, and immune function declines with age. This process, immunosenescence, could alter their effectiveness and their toxicity in older patients, who dominate clinical practice but formed a small and relatively fit minority in the trials. Chapter 5 compared immune-related ADRs between patients below and above 65 years in a single hospital, across all tumor types treated with a PD-(L)1 inhibitor. Data were collected manually, since the study preceded the introduction of text mining. Cutaneous reactions, thyroid disorders and non-infectious diarrhea or colitis occurred most frequently. Most were mild to moderate and none were fatal. Cutaneous reactions were more common in older patients, while no differences were found for the other reactions.

Section III applies the validated methodology in NSCLC. Chapter 6 extended the preceding study to this population alone, added effectiveness as an outcome, and used text mining to assemble a cohort from two hospitals. Effectiveness and the incidence of potentially fatal immune-related adverse events were compared between three age groups. Neither PFS nor overall survival (OS) differed, and the incidence of these reactions was comparable. The concern that immunosenescence would reduce effectiveness or increase toxicity in older patients was not confirmed. With benefit and risk assessed in the same cohort, age alone is not a sufficient basis for withholding treatment.

Therapy-related pneumonitis can interrupt treatment and cause respiratory failure, but its incidence is underestimated in trials and the risk across current regimens was unknown. In Chapter 7, a collaboration study, the incidence of pneumonitis requiring systemic corticosteroids was determined for eleven predefined regimens in stage III and IV NSCLC. It differed substantially between regimens and was highest after consolidation durvalumab, while several chemotherapy and targeted therapy regimens caused no cases at all. Overall incidence exceeded that reported in the pivotal trials. A higher body mass index and more radiation fractions were associated with increased risk. Pneumonitis risk can thus be weighed per regimen at the start of treatment.

The European approval of consolidation durvalumab is restricted to tumors with a PD-L1 tumor proportion score (TPS) ≥1%, based on a post-hoc analysis of the registration trial, whereas the Netherlands reimburses the treatment irrespective of TPS. Chapter 8, a multicenter study, compared survival between durvalumab-treated patients with TPS <1% and TPS ≥1%, and both groups with a historical cohort treated with chemoradiotherapy alone. Patients with TPS <1% derived a modestly smaller benefit, but the difference did not reach statistical significance, and both groups outlived the historical cohort. TPS is measured once before treatment and changes under chemoradiotherapy, which limits its value as a biomarker in this setting. A TPS <1% is not a reliable reason to withhold durvalumab.

ICIs account for a large share of hospital drug expenditure. Section IV examines whether this expenditure can be reduced, and whether treatment in routine care delivers the value on which reimbursement was based. The approved flat dose of pembrolizumab exceeds the dose needed to saturate its target. Weight-based hybrid dosing was introduced on this pharmacokinetic rationale and is now standard practice in the Netherlands, but survival data were lacking. Chapter 9, a collaboration study, compared survival between patients receiving hybrid dosing and standard flat dosing in two hospitals. Hybrid dosing proved non-inferior for OS and PFS, before and after adjustment for confounding, and saved roughly a quarter of the drug per cycle. A lower dose of pembrolizumab yields the same survival at considerably lower cost.

Reimbursement decisions are generally based on trial data, and those trial populations differ from the patients subsequently treated. Chapter 10 determined which patients receiving first-line pembrolizumab for metastatic NSCLC would have qualified for the registration trial, and compared survival and healthcare costs between eligible and ineligible patients. Fewer than half met the trial criteria. Survival among eligible patients was shorter than in the trial itself, and ineligible patients survived shorter still despite comparable PFS. Disease control during treatment was therefore similar, and the difference arose after progression, probably through poorer baseline fitness, competing risks and limited access to second-line treatment. Healthcare costs were similar. These findings illustrate the gap between the trial population and clinical practice, and support systematic use of routine care data to reassess effectiveness and value after reimbursement.

Chapter 11 places these findings in a broader perspective. Text mining reaches a scale that MR cannot, and answers questions that registration trials leave open, but what it retrieves is only as good as what clinicians have written down. Other limitations apply to the studies presented here. Hospital records cover only secondary care, quality of life was not recorded, and the absence of a valid comparator prevented formal cost-effectiveness analysis.

Routine care data make it possible to test the assumptions behind eligibility criteria, label restrictions and registered dosing schedules. This thesis shows that these assumptions do not consistently hold, for reasons that differ from case to case. Applying such evidence structurally requires better clinical documentation, linkage of data at national level, and agreement on which variables must be recorded to allow treatments to be reassessed after reimbursement. Cancer survival in the Netherlands continues to improve, while expenditure on expensive hospital medicines rises rapidly. Determining which patient benefits from which treatment, on the basis of evidence from the patients who actually receive it, is becoming more urgent.
["GET
