The dissertation “Artificial Intelligence for Immune Health Assessments in Radiology - Relevance of the Thymus in Adults” examines, across multiple studies, whether the adult thymus remains an active, measurable, and actionable potential determinant of health throughout life. Leveraging advances in medical imaging AI, an automatic, CT-based biomarker of thymic functionality, termed “thymic health”, was developed that captures imaging characteristics beyond human visual interpretation. As such, thymic health represents a potential novel pillar of precision medicine to support risk stratification and treatment decision making, pending prospective validation and interventional studies to confirm generalizability, establish thresholds, and assess clinical utility.

The dissertation is structured into several parts. The introduction reveals the slowly evolving understanding of thymic relevance in adulthood, the limitations of current methods for assessing thymic function at scale, and the unmet need and opportunity to leverage radiology imaging to assess thymic function. By highlighting the potential of AI-driven imaging biomarkers to overcome current limitations in thymic research, the introduction sets the stage for our investigations of thymic health across diverse cohorts, clinical settings, and diseases.

One key finding of the presented research is its impact on population health. Thymic health varied widely among adults, independent of age and sex, and was independently associated with long-term health and lifespan. Higher thymic health aligned with lower mortality, reduced incidence of lung cancer and cardiovascular disease, and a more favorable inflammatory-metabolic profile.

Another key finding was in Immuno-oncology. In patients receiving immune checkpoint inhibitors (ICI), higher thymic health was associated with improved survival outcomes across tumor types and added host-centric information beyond tumor-centric biomarkers. In parallel, higher thymic health identified patients at greater risk of immune-related adverse events, potentially enabling paired efficacy-toxicity stratification through thymic health in the future. In TRACERx, thymic health correlated with blood-based assays of thymic function, validating its biological basis. Further, in patients treated with hormone therapy for breast and prostate cancer, higher thymic health was associated with improved overall survival, suggesting relevance beyond immunotherapy.

Beyond oncology, key findings extended to refinements of cardiovascular risk assessments. Thymic health was inversely associated with coronary artery calcium (CAC) burden and hypothesis-generating explorations suggest that thymic health may modify CAC’s prognostic value for major cardiovascular events, potentially suggesting a novel high-risk phenotype of individuals who harbor high CAC despite high thymic health, pending validation in independent cohorts.

Overall, the results presented in this thesis support a shift from tumor- or lesion-centric decision-making toward paired assessments of disease biology and host immune competence. Because thymic health is derived from scans patients already undergo, it is zero-burden, cost-neutral, and immediately integrable across multiple diseases, as showcased for population health, ICI outcome monitoring, hormone-therapy risk stratification, and immune-informed CAC interpretation. While our observational analyses lay a strong foundation for thymic relevance throughout life, they cannot establish causality; prospective, interventional studies are needed to test whether preserving or improving thymic health changes outcomes. Future priorities include thymic-health-guided ICI sequencing and toxicity monitoring, and longitudinal imaging-biology studies of modifiable potential drivers of thymic decline, such as smoking, obesity, or metabolic health, and potential rejuvenation strategies.

In conclusion, this thesis demonstrates that the adult thymus remains highly relevant throughout life and that its functional status can be measured opportunistically from routine CT. By leveraging AI to turn thoracic imaging into a biomarker of immune competence, we provide a foundation for a novel field of research, prospective validation, and a new potential biomarker for more precise prevention and care, and a platform to test restorative immune strategies that may help patients live longer, healthier lives in the future.
["GET
