Daarnaast werd verondersteld dat deze processen kunnen worden weerspiegeld door circulerende biomarkers met diagnostische, prognostische en translationele relevantie.\n\nOp basis hiervan was het algemene doel van dit proefschrift om het huidige inzicht in de pathofysiologische mechanismen en klinisch relevante biomarkers over het volledige spectrum van leverziekten te verdiepen, met bijzondere aandacht voor extracellulaire blaasjes, metabole disfunctie-geassocieerde steatotische leverziekte (MASLD) en levertransplantatie.\n\nHoofdstuk 1 introduceert de wereldwijde ziektelast en het veranderende spectrum van chronische leverziekten, met bijzondere nadruk op de toenemende impact van metabole disfunctie-geassocieerde steatotische leverziekte (MASLD) en de systemische aard van leverziekten gedurende de progressie van steatose naar cirrose en uiteindelijk levertransplantatie. Daarnaast worden de huidige diagnostische en prognostische benaderingen besproken, worden bestaande hiaten in biomarkeronderzoek en translationeel onderzoek ge\u00efdentificeerd en wordt het overkoepelende kader van dit proefschrift uiteengezet.\n\nHoofdstuk 2 bespreekt de opkomende rol van extracellulaire blaasjes (EV\u2019s) als natuurlijke dragers van antioxidatieve enzymen, die hun lading beschermen tegen afbraak en gerichte regulatie van de redoxhomeostase in ontvangende cellen mogelijk maken. Tevens wordt het translationele potentieel van zowel natuurlijke als gemodificeerde EV\u2019s bij aandoeningen die gepaard gaan met oxidatieve stress belicht, waarbij aandacht wordt besteed aan belangrijke uitdagingen op het gebied van karakterisering van de lading, beoordeling van enzymatische activiteit, reproduceerbaarheid en klinische opschaalbaarheid.\n\nHoofdstuk 3 beschrijft de dubbele rol van bacteri\u00eble EV\u2019s bij leverziekten. Hierbij wordt ingegaan op hun bijdrage aan leverontsteking, fibrose, metabole ontregeling en tumorvorming, evenals op de beschermende effecten van bacteri\u00eble EV\u2019s afkomstig van commensale en probiotische bacteri\u00ebn. Daarnaast worden de therapeutische mogelijkheden van bacteri\u00eble EV\u2019s besproken en worden de belangrijkste uitdagingen en toekomstige onderzoeksrichtingen geschetst.\n\nHoofdstuk 4 presenteert een verkennende pilotstudie waaruit blijkt dat uit plasma ge\u00efsoleerde EV\u2019s specifieke eiwitsignaturen bevatten die geassocieerd zijn met steatose, steatohepatitis en fibrose bij MASLD. Deze bevindingen ondersteunen het potenti\u00eble gebruik van EV\u2019s als niet-invasieve biomarkers voor ziektestadium en ziekteactiviteit. Daarnaast toont dit hoofdstuk correlaties aan tussen geselecteerde EV-eiwitten en leverbeeldvormingsparameters verkregen met magnetische resonantie (MR), terwijl tevens wordt benadrukt dat validatie in grotere en onafhankelijke cohorten noodzakelijk is.\n\nHoofdstuk 5 laat zien dat geniposidinezuur de proliferatie en activatie van primaire hepatische stellaatcellen van ratten in vitro remt. Mechanistisch hangen deze effecten samen met opregulatie van peroxisoomproliferator-geactiveerde receptor gamma (PPAR\u03b3) en remming van de prote\u00efnekinase B\/glycogeen synthase kinase 3 beta (AKT\/GSK3\u03b2)-signaalroute. Daarnaast worden bescheiden ontstekingsremmende effecten waargenomen in met lipopolysaccharide gestimuleerde Kupffer-cellen.\n\nHoofdstuk 6 bespreekt de wereldwijde ziektelast van MASLD aan de hand van gegevens uit de Global Burden of Disease Study 2021. Op basis hiervan wordt geprojecteerd dat, hoewel de naar leeftijd gestandaardiseerde incidentie- en prevalentiecijfers de komende decennia naar verwachting relatief stabiel blijven, de absolute ziektelast van MASLD verder zal toenemen.\n\nHoofdstuk 7 onderzoekt plasma GlycA als marker voor chronische laaggradige ontsteking bij pati\u00ebnten met cirrose en ontvangers van een levertransplantatie. Er wordt aangetoond dat verhoogde GlycA-concentraties sterk samenhangen met sterfte door alle oorzaken, wat de waarde van GlycA als prognostische biomarker bij gevorderde leverziekte en na levertransplantatie ondersteunt.\n\nHoofdstuk 8 onderzoekt veranderingen in de plasmaconcentraties van vertakte-keten aminozuren (branched-chain amino acids; BCAA\u2019s) bij pati\u00ebnten met cirrose en levertransplantatieontvangers. De resultaten laten zien dat BCAA-spiegels in beide groepen lager zijn dan in de algemene bevolking en toenemen na levertransplantatie. Tevens wordt aangetoond dat hogere BCAA-spiegels bij cirrose geassocieerd zijn met een lagere sterfte door alle oorzaken, terwijl zij bij levertransplantatieontvangers onafhankelijk samenhangen met een verhoogd risico op nieuw ontstane diabetes mellitus type 2.\n\nHoofdstuk 9 toont aan dat circulerende citraatconcentraties sterk verhoogd zijn bij pati\u00ebnten met eindstadium leverziekte en afnemen na levertransplantatie. Dit suggereert dat citraat een weerspiegeling vormt van reversibele metabole en mitochondriale verstoringen die samenhangen met ernstige leverfunctiestoornissen. Daarnaast blijkt dat hogere plasmacitraatconcentraties geassocieerd zijn met een verhoogde sterfte door alle oorzaken bij pati\u00ebnten op de wachtlijst voor levertransplantatie, hetgeen wijst op de potenti\u00eble prognostische waarde van citraat.\n\nHoofdstuk 10 laat zien dat hogere plasmacitraatconcentraties onafhankelijk geassocieerd zijn met een lagere spiermassa en verminderde spierkracht bij levertransplantatieontvangers. Hiermee wordt een verband gelegd tussen een verstoord citraatmetabolisme en een verminderde spiergezondheid na transplantatie. Deze bevindingen suggereren dat circulerend citraat kan dienen als biomarker voor het risico op sarcopenie na levertransplantatie en mogelijk tevens een aangrijpingspunt vormt voor toekomstige interventies.\n\nHoofdstuk 11 toont aan dat circulerende citraatconcentraties sterk verhoogd zijn tijdens ernstige hypothyreo\u00efdie en afnemen na suppletie met schildklierhormoon. Dit wijst erop dat de schildklierfunctie een belangrijke invloed heeft op systemische citraatconcentraties. Daarnaast suggereren de bevindingen dat mitochondriale disfunctie ten gevolge van hypothyreo\u00efdie aan deze stijging ten grondslag kan liggen en dat hiermee rekening moet worden gehouden bij de interpretatie van citraat als biomarker voor ongunstige klinische uitkomsten.\n\nHoofdstuk 12 integreert de bevindingen van dit proefschrift en laat zien dat chronische leverziekte een dynamisch en biologisch verweven proces is, waarin EV-gemedieerde communicatie, ontsteking, fibrogenese, metabole ontregeling en extrahepatische manifestaties gedurende het gehele ziektecontinu\u00fcm nauw met elkaar samenhangen. Tot slot wordt geconcludeerd dat verdere vooruitgang in het onderzoek naar leverziekten afhankelijk zal zijn van de integratie van mechanistische inzichten, validatie van biomarkers en translationele interventiestrategie\u00ebn, met als uiteindelijk doel een betere ziektestratificatie, prognostische beoordeling en ontwikkeling van nieuwe therapeutische mogelijkheden.","summary":"The work presented in this thesis was guided by the overarching hypothesis that liver disease progression and clinical outcomes are shaped by interconnected processes involving extracellular vesicle-mediated communication, inflammation, fibrogenesis, and systemic metabolic dysregulation, and that these processes may be reflected by circulating biomarkers with diagnostic, prognostic, and translational relevance.\n\nOn this basis, the overall aim of this thesis was to advance current understanding of the pathophysiological mechanisms and clinically relevant biomarkers across the spectrum of liver disease, with particular emphasis on extracellular vesicles, MASLD, and liver transplantation.\n\nChapter 1 introduces the global burden and evolving spectrum of chronic liver disease, with particular emphasis on the rising impact of metabolic dysfunction-associated steatotic liver disease (MASLD) and the systemic nature of liver disease across its progression from steatosis to cirrhosis and liver transplantation. It further outlines current diagnostic and prognostic approaches, identifies unmet needs in biomarker discovery and translational research, and defines the overarching framework of this thesis.\n\nChapter 2 reviews the emerging role of extracellular vesicles (EVs) as natural carriers of antioxidant enzymes that protect their cargo from degradation and enable targeted modulation of redox homeostasis in recipient cells. It also highlights the translational potential of both native and engineered EVs in oxidative stress-related disorders, while addressing key challenges related to cargo characterization, enzymatic activity assessment, reproducibility, and clinical scalability.\n\nChapter 3 summarizes the dual roles of bacterial EVs in liver disease, emphasizing their contributions to hepatic inflammation, fibrosis, metabolic dysregulation, and tumorigenesis, as well as the protective effects of bacterial EVs derived from commensal and probiotic bacteria. It further discusses the therapeutic promise of bacterial EVs and outlines major challenges and future directions.\n\nChapter 4 presents a preliminary exploratory study showing that plasma-derived EVs contain distinct protein signatures associated with steatosis, steatohepatitis, and fibrosis in MASLD, supporting their potential utility as non-invasive biomarkers of disease stage and activity. The chapter also demonstrates correlations between selected EV proteins and magnetic resonance (MR)-based liver imaging parameters, while underscoring the need for validation in larger and independent cohorts.\n\nChapter 5 demonstrates that geniposidic acid suppresses the proliferation and activation of primary rat hepatic stellate cells in vitro. Mechanistically, these effects are linked to upregulation of peroxisome proliferator-activated receptor gamma (PPAR\u03b3) and inhibition of protein kinase B\/glycogen synthase kinase 3 beta (AKT\/GSK3\u03b2) signaling, while modest anti-inflammatory effects are also observed in lipopolysaccharide-stimulated Kupffer cells.\n\nChapter 6 comments on the global burden of MASLD using data from the Global Burden of Disease Study 2021 and it projects that, although age-standardized rates may remain relatively stable over the coming decades, the absolute burden of MASLD will continue to increase.\n\nChapter 7 investigates plasma GlycA as a marker of chronic low-grade inflammation in patients with cirrhosis and liver transplant recipients. It demonstrates that elevated GlycA is strongly associated with all-cause mortality, supporting its value as a prognostic biomarker in advanced liver disease and after liver transplantation.\n\nChapter 8 examines alterations in plasma branched-chain amino acids (BCAAs) in patients with cirrhosis and liver transplant recipients, showing that BCAA levels are reduced in both groups relative to the general population and rise after transplantation. It further demonstrates that higher BCAA levels are associated with lower all-cause mortality in cirrhosis, whereas in liver transplant recipients, they are independently associated with a higher risk of new-onset type 2 diabetes.\n\nChapter 9 shows that circulating citrate levels are markedly elevated in patients with end-stage liver disease and decrease after liver transplantation, suggesting that citrate reflects reversible metabolic and mitochondrial disturbances associated with advanced liver dysfunction. It further demonstrates that higher plasma citrate levels are associated with increased all-cause mortality in patients awaiting transplantation, indicating their potential prognostic relevance.\n\nChapter 10 demonstrates that higher plasma citrate levels are independently associated with lower muscle mass and reduced muscle strength in liver transplant recipients, linking altered citrate metabolism to impaired post-transplant muscle health. These findings suggest that circulating citrate may serve as a biomarker of post-transplant sarcopenia risk and potentially as a target for future intervention.\n\nChapter 11 shows that circulating citrate is markedly elevated during profound hypothyroidism and declines after thyroid hormone supplementation, indicating that thyroid status substantially influences systemic citrate concentrations. The findings further suggest that hypothyroidism-related mitochondrial dysfunction may underlie this increase and should be taken into account when interpreting citrate as a biomarker of adverse outcomes.\n\nChapter 12 integrates the findings of this thesis by showing that chronic liver disease is a dynamic and biologically interconnected process involving EV-mediated communication, inflammation, fibrogenesis, metabolic dysregulation, and extrahepatic manifestations across the disease continuum.
