Vanwege hun unieke immuunsuppressieve eigenschappen hebben Treg-cellen aanzienlijke belangstelling gewekt als kandidaten voor adoptieve celtherapie (ACT), met name voor de behandeling van auto-immuunziekten en in de context van allogene transplantatie. Veelbelovende resultaten zijn reeds verkregen in diverse preklinische muismodellen, waarin ACT met Treg-cellen ontsteking en weefselschade bij auto-immuunziekten en orgaantransplantatie effectief verminderde. Klinische studies bij mensen hebben daarnaast gunstige veiligheidsprofielen aangetoond en eerste aanwijzingen gegeven voor therapeutische werkzaamheid. De therapeutische effecten die tot nu toe bij pati\u00ebnten zijn waargenomen, blijven echter beperkter dan de resultaten uit preklinische studies, wat aangeeft dat er nog aanzienlijke ruimte is voor verdere optimalisatie.\n\nEen mogelijke verklaring voor deze beperkte effectiviteit is dat veel huidige ACT-strategie\u00ebn gebruikmaken van polyclonale Treg-celpopulaties. Het Treg-celcompartiment is echter sterk heterogeen en omvat cellen met verschillende fenotypische, functionele en antigeenspecifieke eigenschappen. Daardoor is mogelijk slechts een deel van de toegediende Treg-cellen optimaal geschikt om de immuunresponsen te reguleren die ten grondslag liggen aan een specifieke ziektecontext. Daarnaast vormen de zuiverheid en stabiliteit van therapeutische Treg-celproducten belangrijke uitdagingen, aangezien contaminatie met conventionele T (Tconv)-cellen of het gebruik van instabiele Treg-cellen niet alleen de therapeutische effectiviteit kan verminderen, maar ook de veiligheid in gevaar kan brengen. Een beter begrip van de heterogeniteit van Treg-cellen is daarom essentieel voor de ontwikkeling van effectieve en veilige Treg-celtherapie\u00ebn. De hoofdstukken in dit proefschrift hebben als doel de heterogeniteit van het humane Treg-celcompartiment verder in kaart te brengen en markers te identificeren die therapeutisch relevante Treg-celsubsets onderscheiden.\n\nDe heterogeniteit van het Treg-celcompartiment is al zichtbaar tijdens de ontwikkeling van Treg-cellen in de thymus. In Hoofdstuk 2 onderzochten we de ontwikkeling van humane Treg-cellen in de thymus met behulp van single-cell RNA-sequencing en flowcytometrie, waarbij we verschillende ontwikkelingsstadia tijdens de toewijding aan de Treg-cellijn identificeerden. We beschreven een pre-Treg-celfase gekenmerkt door signalen van TCR-activatie en expressie van lijn-inadequate genen, evenals een transitiefase gekenmerkt door expressie van FOXO1 en CCR7. Daarnaast tonen we aan dat glycoprote\u00efne A33 (GPA33), een molecuul dat eerder werd geassocieerd met stabiele na\u00efeve Treg-cellen, wordt verworven tijdens late stadia van thymische Treg-celontwikkeling, wat suggereert dat GPA33 thymus-afgeleide Treg (tTreg)-cellen markeert die klaar zijn om naar de periferie te migreren. Verder rapporteren we dat de postnatale humane thymus volwassen, recirculerende effector-achtige Treg-cellen bevat die inflammatoire chemokinereceptoren tot expressie brengen en geen CCR7-expressie vertonen. Tezamen benadrukken deze bevindingen dat het thymische Treg-celcompartiment een dynamische en heterogene populatie vormt, met implicaties voor het therapeutisch gebruik van deze cellen.\n\nIn Hoofdstuk 3 breidden we onze analyse van GPA33-expressie uit naar perifere immuuncelpopulaties. GPA33 kwam het sterkst tot expressie binnen het CD4+ T-celcompartiment. De hoogste expressie werd gevonden op na\u00efeve Treg-cellen en CXCR5+ Treg-cellen, evenals op na\u00efeve, folliculaire helper- en centrale geheugen-Tconv-cellen. Binnen het conventionele centrale geheugen-T-cel (Tcm)-compartiment bracht GPA33 verdere functionele heterogeniteit aan het licht: GPA33+ Tcm-cellen vertoonden een relatief rustend en ongedifferentieerd fenotype, terwijl GPA33\u2212 Tcm-cellen vroege kenmerken van effector-differentiatie lieten zien. Deze bevindingen tonen aan hoe fenotypische markers aanvullende lagen van heterogeniteit kunnen onthullen binnen ogenschijnlijk uniforme populaties. Tevens vormen zij de basis voor het onderzoeken van GPA33 als marker voor cellulaire qui\u00ebscentie en stabiliteit in zowel conventionele als regulatoire T-cellen.\n\nVoortbouwend op onze bevindingen uit Hoofdstuk 3 richtte Hoofdstuk 4 zich op niet eerder gekarakteriseerde subset van humane Treg-cellen met eigenschappen die lijken op conventionele Tcm-cellen. Deze cellen worden gekenmerkt door expressie van GPA33 en afwezigheid van de na\u00efeve marker CD45RA. Deze CD25highGPA33+CD45RA\u2212 cellen zijn verder werkelijke Treg-cellen, zoals blijkt uit hun suppressieve capaciteit en demethylering van de FOXP3 CNS2 TSDR. Zij behielden stabiele expressie van FOXP3 en Helios, zelfs onder inflammatoire omstandigheden, vertoonden proliferatieve capaciteit, brachten lymfeklierhomingmoleculen tot expressie en waren in staat te differenti\u00ebren naar effector-achtige GPA33\u2212 Treg-cellen. Samen suggereren deze bevindingen dat het Treg-celcompartiment georganiseerd is in functioneel verschillende maar onderling verbonden cellulaire staten, analoog aan de organisatie van het Tconv-celcompartiment.\n\nTen slotte vertaalde Hoofdstuk 5 deze bevindingen naar een praktische strategie voor de isolatie van stabiele Treg-cellen voor therapeutische toepassing. We tonen aan dat verrijking van CD4\u207aCD25\u207aCD127low\/\u2212 Treg-cellen op basis van GPA33-expressie de isolatie van lijnstabiele FOXP3\u207aHelios\u207a cellen aanzienlijk verbetert. Deze cellen produceren geen inflammatoire cytokinen en behouden hun suppressieve functie na in vitro expansie. Deze aanpak presteert beter dan bestaande strategie\u00ebn die uitsluitend gebaseerd zijn op CD127, CD45RA of behandeling met rapamycine, en kan verder worden verfijnd door aanvullende markers zoals CD226 en TIGIT te gebruiken. Opvallend is dat de gecombineerde toepassing van GPA33 en CD226 de isolatie van stabiele Treg-cellen mogelijk maakt onafhankelijk van CD127-expressie. Samen bieden deze bevindingen een praktisch raamwerk voor de isolatie van hoogwaardige Treg-celproducten voor ACT.","summary":"CD25+FOXP3+ regulatory T (Treg) cells are specialized CD4+ T cells that play a central role in maintaining immune tolerance by preventing excessive or harmful immune responses. Because of their unique immunosuppressive capacities, Treg cells have attracted considerable interest as candidates for adoptive cell therapy (ACT), particularly for the treatment of autoimmune disease and in the context of allogeneic transplantation. Promising results have been obtained in a variety of preclinical murine models, in which Treg cell ACT effectively mitigated inflammation and tissue damage in autoimmune disease and solid organ transplantation. Early clinical trials in humans have further demonstrated favourable safety profiles and provided initial support for therapeutic efficacy. However, the therapeutic effects observed in patients have thus far remained more modest than those achieved in preclinical studies, indicating substantial room for further optimization. \n\nOne possible explanation for this limited efficacy is that many current ACT strategies rely on polyclonal Treg cell populations. However, the Treg cell compartment is highly heterogeneous and comprises cells with distinct wphenotypic, functional and antigen-specific properties. Consequently, only a subset of transferred Treg cells may be optimally suited to regulate the immune responses underlying a given disease context. In addition, the purity and stability of therapeutic Treg cell products represent important challenges, as contamination with conventional T (Tconv) cells or the use of unstable Treg cells may not only reduce therapeutic efficacy, but could also compromise safety. A better understanding of Treg cell heterogeneity is therefore essential for the development of effective and safe Treg cell-based therapies. The chapters in this thesis aim to further define the heterogeneity of the human Treg cell compartment and to identify markers that distinguish therapeutically relevant Treg cell subsets.\n\nThe heterogeneity of the Treg cell compartment is already apparent during Treg cell development in the thymus. In Chapter 2, we investigated human thymic Treg cell development using single-cell RNA sequencing and flow cytometry, revealing discrete developmental stages during commitment to the Treg cell lineage. We identified a pre-Treg cell phase marked by signs of TCR signalling and the expression of lineage-inappropriate genes, and a transitional stage characterized by FOXO1 and CCR7 expression. We further show that glycoprotein A33 (GPA33), a molecule previously associated with stable na\u00efve Treg cells, is acquired during late stages of thymic Treg cell development, suggesting that GPA33 marks tTreg cells poised for migration into the periphery. In addition, we report that the postnatal human thymus contains mature, recirculating effector-like Treg cells, expressing inflammatory chemokine receptors and lacking CCR7 expression. Together, these findings highlight that the thymic Treg cell compartment already comprises a dynamic and heterogeneous population, with implications for their therapeutic sourcing.\n\nIn Chapter 3, we extended our analysis of GPA33 expression to peripheral immune cell populations. GPA33 was most prominently expressed by cells in the CD4+ T cell compartment. Here, highest expression of GPA33 was found on na\u00efve Treg cells and CXCR5+ Treg cells, as well as by na\u00efve, follicular helper and central memory Tconv cells. Within the conventional central memory T cell (Tcm) compartment, GPA33 revealed further functional heterogeneity, where GPA33+ Tcm displayed a relatively quiescent and undifferentiated phenotype, whereas GPA33\u2212 Tcm cells displayed early features of effector differentiation. These findings demonstrate how phenotypic markers can uncover additional layers of heterogeneity within seemingly uniform populations. They further lay the foundation for exploring GPA33 as a marker of cellular quiescence and stability in both conventional and regulatory T cells. \n\nBuilding on our findings in Chapter 3, Chapter 4 focused on a previously uncharacterized subset of human Treg cells with features resembling conventional Tcm cells. These cells are defined by GPA33 expression and lack of the na\u00efve marker CD45RA. These CD25highGPA33+CD45RA\u2013 cells are genuine Treg cells, as evidenced by their suppressive capacity and demethylation of the FOXP3 CNS2 TSDR. They retained stable FOXP3 and Helios expression, even under inflammatory conditions, showed proliferative capacity, expressed lymph node-homing molecules, and demonstrated the ability to differentiate into effector-like GPA33\u2013 Treg cell states. Together, these findings suggest that the Treg cell compartment is organized into functionally distinct yet interconnected cellular states, analogous to the organization of the Tconv cell compartment.\n\nFinally, Chapter 5 translated these observations into a practical strategy for the isolation of stable Treg cells for therapeutic application. We demonstrate that enrichment of CD4+CD25+CD127low\/\u2013 Treg cells based on GPA33 expression substantially improves the isolation of lineage-stable FOXP3+Helios+ cells that refrain from producing inflammatory cytokines and maintain suppressive function after in vitro expansion. This approach outperforms existing strategies relying on CD127, CD45RA, or rapamycin treatment alone, and could be further refined by through the inclusion of additional markers such as CD226 and TIGIT. Notably, the combined use of GPA33 and CD226 enabled the isolation of stable Treg cells independently of CD127.
