Ondanks deze aanzienlijke ziektelast zijn farmacologische strategie\u00ebn die de hepatische fibrogenese effectief kunnen remmen of omkeren nog steeds zeer beperkt beschikbaar. Om aan deze onvervulde klinische behoefte tegemoet te komen, is een diepgaand begrip nodig van de cellulaire en moleculaire mechanismen die fibrose binnen de hepatische micro-omgeving aandrijven. Dit proefschrift onderzoekt deze mechanismen langs twee complementaire dimensies, namelijk celintrinsieke metabole herprogrammering en intercellulaire communicatie via extracellulaire vesikels (EV\u2019s), met als overkoepelend doel het identificeren van exploiteerbare moleculaire kwetsbaarheden in de belangrijkste celpopulaties die de progressie van MASLD bepalen.\n\nHoofdstuk 1 biedt het conceptuele fundament voor het experimentele werk door de huidige kennis van het hepatische multicellulaire netwerk in gezondheid en ziekte samen te vatten. Ten tweede zijn het moleculaire cargo en de functionele gevolgen van EV-gemedieerde signalering tussen hepatische niet-parenchymale celpopulaties onvoldoende gekarakteriseerd, met name wat betreft hun vermogen om fibrogene activatie te versterken dan wel op te heffen.\n\nHoofdstuk 2 richt zich op de parenchymale dimensie van MASLD door te onderzoeken hoe groeidifferentiatiefactor 15 (GDF15) hepatocyten beschermt tegen lipotoxische schade. Deze bevindingen positioneren de GDF15-ERK1\/2-mitochondri\u00eble dynamiek-as als een potentieel therapeutisch doelwit voor het herstel van de metabole competentie van hepatocyten tijdens vroege MASLD.\n\nHoofdstuk 3 verlegt de focus naar het niet-parenchymale compartiment en onderzoekt hoe pirfenidon de activatie van HSC\u2019s onderdrukt door het aminozuurmetabolisme-programma aan te pakken dat de fibrogene functie ondersteunt. Lipopolysaccharide (LPS) verhoogt fibrogene markers in HSC\u2019s niet rechtstreeks in monocultuur, maar versterkt de HSC-activatie aanzienlijk wanneer KC\u2019s aanwezig zijn, wat aangeeft dat de profibrotische gevolgen van TLR4-signalering via KC\u2019s worden gemedieerd in plaats van via directe TLR4-activering op HSC\u2019s. KC-afgeleide EV\u2019s zijn voldoende om de expressie van alfa-gladspieractine (\u03b1-SMA) in HSC\u2019s te verhogen, zelfs onder basale omstandigheden, en LPS-geprimegde KC-afgeleide EV\u2019s reguleren bovendien de expressie van pro-inflammatoire cytokinen in HSC\u2019s, wat aantoont dat de samenstelling van het EV-cargo dynamisch wordt bepaald door de activatiestatus van de oudercel. Omgekeerd geven vroeggeactiveerde HSC\u2019s EV\u2019s af die KC\u2019s polariseren naar een pro-inflammatoir fenotype. Onbevooroordeelde proteomische analyse identificeert katalase als een belangrijk bestanddeel van het LSEC-afgeleide EV cargo, en functionele validatie met PEG-katalase bevestigt dat intracellulaire afgifte van katalase het senescentie-fenotype reproduceert. Mechanistisch gezien bevordert katalase de mitochondri\u00eble superoxideproductie in geactiveerde HSC\u2019s en activeert de TLR4\/NF-\u03baB-signalering, die het stroomafwaartse senescentieprogramma aandrijft. Deze schijnbaar paradoxale pro-oxidatieve werking van een antioxidantenzym is verenigbaar met het vermogen van katalase om hydroxylradicalen te genereren via Fenton-typische chemie in cellen met verhoogde labiele ijzerpools en actief NADPH-oxidase, zoals kenmerkend is voor het geactiveerde HSC-fenotype. Despite this substantial disease burden, pharmacological strategies that effectively halt or reverse hepatic fibrogenesis remain critically limited. Addressing this unmet clinical need requires a comprehensive understanding of the cellular and molecular mechanisms that drive fibrosis within the hepatic microenvironment. This thesis investigates these mechanisms along two complementary dimensions, namely cell-intrinsic metabolic reprogramming and extracellular vesicle (EV)-mediated intercellular communication, with the overarching aim of identifying exploitable molecular vulnerabilities in the key cell populations that govern MASLD progression.\n\nChapter 1 provides the conceptual foundation for the experimental work by reviewing the current understanding of the hepatic multicellular network in health and disease. It describes how hepatocytes, hepatic stellate cells (HSCs), Kupffer cells (KCs), and liver sinusoidal endothelial cells (LSECs) undergo pathological transformation during MASLD, and how their coordinated dysfunction collectively sustains the fibroinflammatory milieu. The chapter identifies two critical knowledge gaps that motivated the subsequent investigations. First, the precise intracellular mechanisms by which hepatocytes and activated HSCs rewire their metabolic programs under disease-relevant stress, and whether these adaptations create targetable vulnerabilities, remain incompletely defined. Second, the molecular cargo and functional consequences of EV-mediated signaling between hepatic non-parenchymal cell populations are insufficiently characterized, particularly with respect to their capacity to either amplify or resolve fibrogenic activation.\n\nChapter 2 addresses the parenchymal dimension of MASLD by investigating how growth differentiation factor 15 (GDF15) protects hepatocytes against lipotoxic injury. Using primary rat hepatocytes and human liver organoids, this chapter demonstrates that exposure to the saturated fatty acid palmitate shifts the mitochondrial fusion-fission equilibrium toward excessive fragmentation, resulting in impaired fatty acid translocation and diminished fatty acid oxidation capacity. GDF15 selectively reverses this morphodynamic imbalance under lipotoxic conditions by restoring the expression of the fusion mediators mitofusin 1 (MFN1) and optic atrophy 1 (OPA1) while reducing the fission effector dynamin-related protein 1 (DRP1). Mechanistically, GDF15 exerts these protective effects through the inhibition of ERK1\/2 signaling. This chapter reveals that activated HSCs upregulate an activating transcription factor 4 (ATF4)-dependent biosynthetic program encompassing glutaminase 1 (GLS1), serine hydroxymethyltransferase 2 (SHMT2), and pyrroline-5-carboxylate synthase (P5CS). These enzymes collectively support glutamine catabolism, serine-glycine one-carbon metabolism, and de novo proline synthesis to meet the dual demands of rapid proliferation and high-rate collagen matrix production. Pirfenidone inhibits ATF4 nuclear translocation and reduces the protein levels of all three enzymes, thereby simultaneously restricting collagen precursor availability and depleting glutathione (GSH). This combined metabolic disruption renders activated HSCs unable to maintain the redox buffering capacity required for antioxidant defense. Consequently, pirfenidone pretreatment markedly sensitizes activated HSCs to ferroptosis upon co-treatment with the system Xc inhibitor erastin. The selectivity of this combination strategy is supported by the differential dependence of activated HSCs and hepatocytes on xCT-mediated cystine import for cysteine supply, as hepatocytes predominantly rely on the trans-sulfuration pathway for GSH biosynthesis. These findings establish a mechanistic framework in which the pharmacological dismantling of a metabolic survival program creates an exploitable vulnerability analogous to the synthetic lethality principle applied in oncology.\n\nChapter 4 extends the investigation from individual cellular metabolism to the intercellular communicative layer by characterizing the bidirectional EV-mediated crosstalk between KCs and HSCs. Using primary cell co-culture systems, this chapter demonstrates that KC and HSC activation are mutually reinforcing through a Toll-like receptor 4 (TLR4)-dependent mechanism. Lipopolysaccharide (LPS) does not directly enhance HSC fibrogenic markers in monoculture but markedly amplifies HSC activation when KCs are present, indicating that the pro-fibrotic consequences of TLR4 signaling are mediated by KCs rather than through direct TLR4 engagement on HSCs. KC-derived EVs are sufficient to increase alpha-smooth muscle actin (\u03b1-SMA) expression in HSCs even under basal conditions, and LPS-primed KC-derived EVs additionally upregulate pro-inflammatory cytokine expression in HSCs, demonstrating that the EV cargo composition is dynamically shaped by the activation state of the parent cell. Reciprocally, early-activated HSCs release EVs that polarize KCs toward a pro-inflammatory phenotype. These findings delineate a self-reinforcing fibroinflammatory circuit in which each cell type continuously amplifies the pathological activation of the other through vesicular cargo exchange. The TLR4 inhibitor TAK-242 effectively suppresses this bidirectional activation, identifying TLR4 signaling as a druggable node within this circuit.\n\nChapter 5 completes the intercellular communication axis by investigating the opposing, anti-fibrotic dimension of EV-mediated signaling within the hepatic sinusoid. This chapter reveals that LSEC-derived EVs carry catalase as a functionally significant cargo molecule and that these EVs induce cellular senescence rather than fibrogenic activation in HSCs. Senescence induction is evidenced by elevated senescence-associated beta-galactosidase (SA-\u03b2-gal) activity, upregulation of the cell cycle inhibitor p21, acquisition of a senescence associated secretory phenotype (SASP) including interleukin-1 beta (IL-1\u03b2), and suppression of proliferation. Unbiased proteomic analysis identifies catalase as a major constituent of the LSEC-derived EV cargo, and functional validation using PEG-catalase confirms that intracellular catalase delivery recapitulates the senescence phenotype. Mechanistically, catalase promotes mitochondrial superoxide production in activated HSCs and activates TLR4\/NF-\u03baB signaling, which drives the downstream senescence program. This seemingly paradoxical pro-oxidant activity of an antioxidant enzyme is consistent with the capacity of catalase to generate hydroxyl radicals through Fenton-type chemistry in cells with elevated labile iron pools and active NADPH oxidase, as is characteristic of the activated HSC phenotype. These findings identify the LSEC-derived catalase-EV axis as an endogenous anti-fibrotic mechanism that inactivates fibrogenic HSCs through the imposition of an irreversible senescent fate.\n\nChapter 6 integrates the findings from the preceding experimental chapters into a unified conceptual framework. It discusses how the cell-intrinsic metabolic reprogramming events described in Chapters 2 and 3 generate both the pathological cellular states and the therapeutic vulnerabilities that define the MASLD microenvironment, while the EV-mediated intercellular communication networks characterized in Chapters 4 and 5 determine whether those cellular states are amplified toward disease progression or redirected toward resolution. The discussion highlights the functional duality of the hepatic microenvironment, in which a pro-fibrotisch KC-HSC vesicular circuit and an anti-fibrotic LSEC-HSC vesicular axis coexist and compete to shape the net trajectory of fibrogenesis. Chapter 6 also addresses the methodological strengths and limitations of the experimental approach, including the advantages of primary cell co-culture systems and human liver organoids, the absence of comprehensive in vivo validation, the heterogeneity of EV populations, and the exclusive use of male-derived primary cells. Finally, it outlines translational perspectives, including the development of GDF15-based therapies for early MASLD, HSC-targeted combination strategies exploiting ferroptosis sensitization, EV-based liquid biopsy diagnostics, and engineered nanoparticle delivery of catalase to promote HSC senescence.\n\nCollectively, this thesis demonstrates that the progression and potential resolution of liver fibrosis are governed by the interplay between cell-intrinsic metabolic adaptations and intercellular vesicular communication within the hepatic sinusoidal microenvironment. By elucidating how lipotoxic hepatocytes fail metabolically, how activated HSCs construct and depend upon a targetable biosynthetic program, how KCs and HSCs lock each other into pathological activation through vesicular exchange, and how LSECs deploy catalase-rich EVs to impose senescence on fibrogenic cells, this work identifies multiple mechanistically defined intervention points.
