Op groepsniveau is hoog gedoseerd intraveneus immunoglobuline (IVIg) een bewezen effectieve behan deling bij pati\u00ebnten met GBS en CIDP. Pati\u00ebnten krijgen een inductietherapie van 2 g\/kg IVIg toegediend over 2-5 dagen. Pati\u00ebnten met CIDP hebben daarnaast vaak onderhoud stherapie nodig met een variabele dosering en frequentie van IVIg of subcutane immu noglobulinen (SCIg). Een aanzienlijk deel van de pati\u00ebnten reageert echter onvoldoende of houdt restverschijnselen en klachten. Om de huidige therapie te verbeteren en richt ing model-ge\u00efnformeerde precisiedosering (MIPD), we hebben we in dit proefschrift de volgende doelen gesteld: a) beschrijven van de farmacokinetiek (PK) van IVIg bij GBS en CIDP, b) koppelen van de PK aan de farmacodynamiek (PD), en c) het identificeren van nieuwe biomarkers om de PK van IVIg beter te begrijpen.\n\nIn Hoofdstuk 1 werden de huidige uitdagingen in de behandeling van pati\u00ebnten met GBS en CIDP beschreven en werd de rationale voor een populatie PK(PD)-benadering ge\u00efntroduceerd. In Hoofdstuk 2 werd middels een systematische review gekeken naar populatie PK(PD) modellen voor IVIg en\/of SCIg bij verschillende indicaties. Gemeen schappelijke kenmerken van deze modellen zijn; een twee-compartimentmodel met een klein verdelingsvolume en een effect van gewicht op klaring en volume.\n\nHet doel van Hoofdstuk 3 was het ontwikkelen en extern valideren van een populatie PK model voor IVIg bij GBS, het identificeren van covariaten die IgG klaring be\u00efnvloeden, en het verkennen van doseringsschema\u2019s die de blootstelling verbeteren. We analyseerden twee grote cohorten van pati\u00ebnten met GBS (177 pati\u00ebnten per cohort) en beschreven het eerste model voor een standaard IVIg kuur bij GBS (0.4 g\/kg gedurende 5 dagen). De data werd het beste beschreven aan de hand van een twee-compartimentmodel met lineaire eliminatie, met een typisch patroon van een snelle piek na voltooiing van de eerste IVIg kuur, gevolgd door redistributie naar het extravasculaire compartiment en daaropvolgende eliminatie. Ernstigere ziekte, uitgedrukt in een hogere GBS-disability score (GBS-DS), verhoogde de IgG klaring significant met ~22% bij pati\u00ebnten die rolstoel of bedgebonden waren (GBS-DS 3\u20134) en met ~83% bij beademde pati\u00ebnten (GBS-DS 5), wat consistent is met een meer katabole of inflammatoire toestand. Gelijktijdige toediening van hooggedoseerd methylprednisolon ging gepaard met ~31% hogere klaring, mogelijk door onderdrukte IgG synthese en verminderde FcRn-gemedieerde recycling. Het model presteerde robuust in externe validatie en maakte klinisch relevante doseringssimulaties mogelijk. We concludeerden dat ernstigere ziekte en toevoeging van methylprednisolon geassocieerd zijn met versnelde IgG klaring bij GBS, wat de blootstel ling tijdens de actieve ziektefase verlaagt. Het gevalideerde model biedt een belangrijk hulpmiddel om IVIg-dosering te personaliseren en om trials te ontwerpen gericht op het handhaven van een bepaalde doel IgG concentratie.\n\nHoofdstuk 4 richtte zich op de relatie tussen de PK van IVIg en de klinische uitkomst (PD). Het doel was het beoordelen van de PK en PD van zowel een enkele standaard IVIg kuur en een tweede IVIg kuur (SID). Data werd verkregen vanuit de SID-GBS-trial, een gerandomiseerde, placebo gecontroleerde studie waarin pati\u00ebnten met een Modified Erasmus GBS Outcome Score (mEGOS) \u22656 een week na de start van de eerste IVIg kuur werden gerandomiseerd naar SID of placebo, terwijl pati\u00ebnten met mEGOS <6 zonder randomisatie werden gevolgd. Het in Hoofdstuk 3 ontwikkelde model voorspelde totale IgG concentraties na SID nauwkeurig, en SID verhoogde serum-IgG gedurende minstens drie weken vergeleken met placebo en niet-gerandomiseerde pati\u00ebnten. Lagere \u0394IgG op 0\u20132 weken en lagere model-voorspelde IgG-blootstelling (AUC) waren geassocieerd met vertraagde mogelijkheid om zelfstandig te lopen, wat een dosis-responsrelatie bevestigt. Echter, na correctie voor eerder beschreven prognostische klinische variabelen (individu ele componenten van mEGOS) had IgG-blootstelling geen aanvullende prognostische waarde. Bovendien verbeterden pati\u00ebnten in de laagste blootstelling strata niet anders met SID versus placebo. Belangrijk is dat ernstige bijwerkingen, waaronder trombo-em bolische events, vaker voorkwamen na SID en voornamelijk bij pati\u00ebnten met lagere IVIg blootstelling. Deze bevindingen toonden aan dat 1) het ontwikkelde model de PK van een tweede IVIg-kuur nauwkeurig voorspelt, 2) dat lage IgG concentraties geassocieerd zijn met een slechte prognose, en 3) dat een tweede kuur IgG concentraties effectief verhoogt, maar dit niet lijkt te vertalen naar betere klinische uitkomsten.\n\nIn Hoofdstuk 5 werd onderzocht of een vergelijkbare blootstelling-responsrelatie bestaat tijdens onderhoudstherapie bij CIDP. Met gebruik van dal IgG concentraties uit de ProCID-trial (RCT die randomiseerde naar 0.5 vs. 1.0 vs. 2.0 g\/kg IVIg elke 3 weken na een inductie van 2 g\/kg) pasten we een twee-compartiment PK model toe om indi viduele blootstelling (AUC) en \u0394IgG te schatten en deze te relateren aan de INCAT-score, knijpkracht en MRC-somscore (MRC-SS). Dal IgG concentratie en blootstelling namen dosisafhankelijk toe. Interindividuele variabiliteit was aanzienlijk, terwijl intra-individuele variabiliteit in steady state laag was. Na 24 weken waren de mediane IgG dalspiegels 13.6, 16.1 en 19.2 g\/L in respectievelijk de 0.5, 1.0 en 2.0 g\/kg groepen. Echter, noch AUC, \u0394IgG, noch dal IgG correleerden met verbetering op INCAT, knijpkracht of MRC-SS in ongecorrigeerde of gecorrigeerde analyses. Bovendien konden pati\u00ebnten die protocol gedefinieerde rescuebehandeling kregen (geen verbetering v\u00f3\u00f3r week 6 of verslechter ing tijdens de studie) niet consequent worden ge\u00efdentificeerd op basis van lagere IgG blootstelling. Deze bevindingen suggereren dat behandelrespons bij CIDP afhangt van factoren buiten blootstelling, wat de noodzaak ondersteunt voor alternatieve biomarkers om doseringsstrategie\u00ebn te individualiseren.\n\nEen beperking van eerdere studies naar IgG concentraties is dat het niet mogelijk was om therapeutisch IVIg te onderscheiden van endogeen IgG. Om dit op te lossen, hebben we in Hoofdstuk 6 een ELISA beschreven die het mogelijk maakt om exogeen IVIg te kwantificeren via IgG1 allotypen die aanwezig zijn in het IVIg preparaat maar afwezig in de endogene fractie. Longitudinale data van 27 pati\u00ebnten toonden over het algemeen vergelijkbare kinetiek voor exogeen IVIg vergeleken met totaal IgG. Dit gold echter niet voor elke pati\u00ebnt, wat suggereert dat extra factoren endogene en exogene IgG kinetiek be\u00efnvloeden en dat onze aanname van een constante basale endogene IgG concentratie niet altijd correct is. Belangrijk is dat de allotype-samenstelling tussen IVIg-merken vers childe, terwijl variabiliteit tussen batches relatief laag was. Deze methode zal een belan grijk hulpmiddel zijn om de variabiliteit in IVIg PK en behandelrespons beter te begrijpen.\n\nIn Hoofdstuk 7 onderzochten we of variatie in FCGR2\/3 (single nucleotide polymorfis men, SNP; en copy number varianten, CNV\u2019s) geassocieerd was met het ontstaan van GBS, de ernst van de ziekte, de klinische uitkomst, en\/of de PK van IVIg. Middels multi plex ligation-dependent probe amplification (MLPA) werd de genetische variatie in het FCGR2\/3 locus in 467 GBS-pati\u00ebnten versus 919 gezonde controles vergeleken. Er werden geen algemene verschillen in variantfrequenties, geen associaties met tijd tot zelfstandig lopen of met \u0394IgG op 1\u20132 weken na IVIg gevonden. Na stratificatie voor Campylobacter jejuni serologie kwam een FCGR3A duplicatie (\u22653 kopie\u00ebn) vaker voor in pati\u00ebnten met een voorafgaande C. jejuni infectie versus controles, en deze duplicatie was geassocieerd met ernstigere zwakte binnen pati\u00ebnten. Dit wijst mogelijk op versterkte Fc-gemedieerde effectoractiviteit bij postinfectieuze ziekte. Daarnaast werd een zeldzame nieuwe dupli catie (CNR5) in FCGR3A\/3B waargenomen bij \u00e9\u00e9n pati\u00ebnt en verder onderzoek naar de functionele consequenties hiervan is nodig. In algemene zin verklaarde genetische varia tie in Fc\u03b3-receptoren geen verhoogd risico op het ontwikkelen van GBS of variabiliteit in de PK van IVIg, maar associaties kunnen bestaan in specifieke subgroepen.\n\nIn Hoofdstuk 8 werd de dynamiek van serum neurofilament light chain (NfL), een marker voor axonale schade, beschreven bij 281 SID-GBS pati\u00ebnten. NfL was verhoogd bij start van behandeling vergeleken met gezonde controles, steeg verder in de eerste 2-4 weken en begon te dalen richting 3 maanden, wat het monophasische beloop van GBS weer spiegelt. Hogere NfL concentraties waren geassocieerd met ernstigere ziekte, en NfL was een onafhankelijke voorspeller van slechte uitkomst. Opvallend was dat NfL dynamiek niet verschilde tussen pati\u00ebnten gerandomiseerd voor een tweede dosis IVIg of placebo. Dit is in lijn met de resultaten van de SID-GBS studie waarin pati\u00ebnten die een tweede IVIg kuur kregen niet sterker verbeterden dan pati\u00ebnten die placebo kregen. Serum NfL is in potentie een biomarker om axonale schade te monitoren en kan dienen als eindpunt voor toekomstige studies naar inflammatoire neuropathie\u00ebn.\n\nIn Hoofdstuk 9 werden de resultaten van de voorgaande hoofdstukken bediscussieerd en in context geplaatst. Het doel van dit proefschrift was om te werken aan betere, gepersonaliseerde behandeling voor pati\u00ebnten met inflammatoire neuropathie\u00ebn. Vooruitgang in het monitoren van IgG, de totstandkoming van populatie PK modellen voor IVIg in inflammatoire neuropathie\u00ebn, en het beschrijven van (nieuwe) biomarkers zal bijdragen aan geoptimaliseerde IVIg dosering en nauwkeurigere prognose bij GBS en CIDP. Meer onderzoek is nodig, vooral naar de relatie tussen PK en PD, om uiteindelijk een effectievere en duurzamere IVIg therapie voor deze pati\u00ebnten te realiseren.","summary":"ENGLISH SUMMARY\n\nGuillain\u2013Barr\u00e9 syndrome (GBS) and chronic inflammatory demyelinating polyradiculo neuropathy (CIDP) are immune\u2011mediated neuropathies with substantial heterogeneity in clinical presentation, disease course, and response to therapy. On a group level, high-dose intravenous immunoglobulin (IVIg) is a proven effective treatment in patients with GBS and CIDP. Patients receive an induction therapy of 2g\/kg IVIg given over 2-5 days. Patients with CIDP often require maintenance therapy with a variable dosage and frequency of IVIg or subcutaneous immunoglobulins (SCIg). However, a substantial proportion of patients respond insufficiently or retain residual deficits and complaints. To move beyond standard weight\u2011based regimens toward model\u2011informed precision dosing (MIPD), we addressed the following aims in this thesis: a) describe the pharmacokinetics (PK) of IVIg in GBS and CIDP, b) link the PK of IVIg to the pharmacodynamics (PD), and c) to identify novel genetic and fluid biomarkers to better understand the PK of IVIg.\n\nIn Chapter 1, the current challenges in the treatment of patients with GBS and CIDP are outlined and the rationale for a PK-PD approach in these patients is motivated. In Chapter 2, we systematically reviewed all current population PK(PD) models of IVIg and\/or SCIg across indications. Common features include a two\u2011compartment model with low volume of distribution and effect of weight on clearance and volume.\n\nThe aim of the study in Chapter 3 was to develop and externally validate a population PK model for IVIg in GBS, identify covariates that influence IgG clearance, and explore dosing regimens that improve exposure. We analyzed two large cohorts of patients with GBS (177 patients in each cohort) and built the first model of a standard IVIg course in GBS (0.4g\/kg for 5 days). A two\u2011compartment model with linear elimination best described the data, with the typical pattern of a rapid peak after completion of the first course followed by redistribution to extravascular compartments and subsequent elimination. More severe disease, expressed by a higher GBS\u2011disability score (GBS-DS), significantly increased IgG clearance by ~22% in patients who were chairbound or bedbound (GBS\u2011DS 3-4) and by ~83% in mechanically ventilated patients (GBS\u2011DS 5), consistent with a more catabolic or inflammatory state. Concomitant high\u2011dose methylprednisolone was associated with ~31% higher clearance, potentially due to suppressed IgG synthesis and reduced FcRn\u2011mediated recycling. The model performed robustly in external validation and enabled clinically relevant dosing simulations. We concluded that a more severe disease and addition of methylprednisolone is associated with an accelerated IgG clearance in GBS, lowering exposure during the active disease phase. The validated model provides an important tool to personalize IVIg dosing and to design trials aimed at maintaining a certain target IgG concentration.\n\nChapter 4 focused on how the PK of IVIg relates to clinical outcome (PD). The aim of the study was to assess the PK and PD of a single standard IVIg course and a second IVIg course (SID). Data was used from the SID\u2011GBS trial, a randomized controlled trial in which patients with a modified Erasmus GBS outcome score (mEGOS) \u2265 6 after the first IVIg course were randomized to receive SID or placebo, while patients with mEGOS < 6 were followed without randomization. The model developed in Chapter 3 accurately predicted total IgG concentrations after SID, and SID increased serum IgG for at least three weeks compared to placebo and the non\u2011randomized patients. Lower \u0394IgG at 0\u20132 weeks and lower model\u2011predicted IgG exposure (area under the curve: AUC) were associated with delayed ability to walk unaided, confirming a dose-response relationship. However, when adjusted for established prognostic clinical variables (individual components of mEGOS), IgG exposure did not have an additional prognostic value. Moreover, patients in the lowest exposure strata did not improve differently with SID versus placebo. Importantly, serious adverse events, including thromboembolic events, were more frequent after SID, and occurred predominantly in patients with lower IVIg exposure rather than higher concentrations. Together, these findings show that the developed model accurately predicts the pharmacokinetics of a second IVIg course, confirm that low IgG levels are associated with poor prognosis, demonstrate that a second course effectively raises IgG concentrations, but also reveal that this increase does not translate into improved clinical outcomes.\n\nIn Chapter 5 the aim was to explore whether a similar exposure\u2013response relationship exists during maintenance therapy in patients with CIDP. Using trough IgG concentrations from the ProCID trial (RCT that randomized to 0.5 vs 1.0 vs 2.0 g\/kg IVIg every 3 weeks after a 2 g\/kg induction), we applied a two\u2011compartment population PK model to estimate individual exposure (AUC) and \u0394IgG and related these to the Inflammatory Neuropathy Cause and Treatment (INCAT) score, grip strength, and Medical Research Council sum score (MRC-SS). Trough IgG concentration and exposure increased dose\u2011dependently. Inter\u2011individual variability was substantial, whereas intra\u2011individual variability at steady state was low. At week 24, median IgG trough concentrations were 13.6, 16.1, and 19.2 g\/L in the 0.5, 1.0, and 2.0 g\/kg groups, respectively. However, neither AUC, \u0394IgG, nor trough IgG correlated with improvement on INCAT, grip strength, or MRC\u2011SS in unadjusted or adjusted analyses. In addition, patients who received protocol\u2011defined rescue treatment (not improving before week 6 or worsening on IVIg during the study period) were not consistently identifiable by lower IgG exposure. These findings suggest that treatment response depends on factors beyond exposure in CIDP, supporting the need for alternative biomarkers to individualize dosing strategies in patients with CIDP.\n\nA limitation apparent in previous studies regarding IgG concentrations is that it is not possible to distinguish therapeutic IVIg from endogenous IgG. Therefore, in Chapter 6 an ELISA targeting IgG1 allotypes was described to quantify exogenous IVIg in recipients lacking allotypes that are present in the exogenous fraction. Overall, longitudinal data from 27 patients showed similar kinetics for exogenous IVIg as compared to total IgG. However, this was not the case for every patient, suggesting that additional factors influence endogenous and exogenous IgG kinetics, and our assumption of a constant baseline endogenous IgG concentration may not always be correct. Importantly, the allotype composition between IVIg brands differed, whereas inter\u2011batch variability was relatively low. This method will be an important tool to better understand the variability in IVIg PK and treatment response of all patients treated with IVIg.\n\nIn Chapter 7 we examined whether FCGR2\/3 variation (single nucleotide polymorphisms, SNPs; and copy number variants, CNVs) influences GBS susceptibility, severity, outcomes, or IVIg PK. Using multiplex ligation-dependent probe amplification (MLPA) we tested 467 patients with GBS versus 919 controls. There were no overall differences in variant frequencies, and no associations with time to walk unaided or with \u0394IgG at 1\u20132 weeks after IVIg treatment. However, after stratification for Campylobacter jejuni positive serology, FCGR3A duplication (\u22653 copies) was enriched versus controls and associated with more severe weakness, suggesting amplified Fc\u2011mediated effector activity in post\u2011infectious disease. In addition a rare novel duplication (CNR5) in FCGR3A\/3B was observed in one patient, and further research is required on the functional consequences of this CNR5 duplication. Overall, Fc\u03b3R genetics do not explain general GBS risk or IVIg PK variability, but associations may exist in specific subgroups.\n\nIn Chapter 8 serum neurofilament light (NfL), a marker for axonal damage, was measured in 281 SID\u2011GBS patients at multiple time points throughout the disease. NfL was elevated at the start of treatment versus healthy controls, increased further over the first 2-4 weeks, and started to decline toward 3 months, mirroring the monophasic course of GBS. Higher NfL concentrations were associated with more severe disease and NfL was an independent predictor of poor outcome. Notably, NfL dynamics did not differ between second\u2011dose IVIg and placebo, aligning with the result in the SID-GBS trial where no benefit was observed when patients were given a SID versus placebo. Together, serum NfL offers a biomarker to monitor axonal damage and may be used as an endpoint for future GBS treatment studies.\n\nIn Chapter 9 all previous chapters were discussed and contextualized. The goal of this thesis was to move towards better, personalized treatment for patients with inflammatory neuropathies. Advances in IgG monitoring, PK-PD modeling, and biomarker discovery will contribute to optimized IVIg dosing and more accurate prognosis in patients with GBS and CIDP.
