De aandoening gaat vaak gepaard met hevig menstrueel bloedverlies, pijnlijke menstruaties (dysmenorroe), chronische bekkenpijn en subfertiliteit en heeft een grote impact op de kwaliteit van leven. Ondanks de hoge prevalentie bestaan er nog veel onduidelijkheden over de pathogenese, diagnostiek en optimale behandeling van adenomyose. Met de opkomst van transvaginale echografie (TVS) en MRI is de diagnostiek sterk verbeterd, maar uniforme criteria voor diagnose en classificatie van ernst ontbreken nog. Daarnaast zijn de therapeutische mogelijkheden beperkt en blijft een baarmoederverwijdering (hysterectomie) vaak over als behandeling als medicamenteuze therapie faalt. Minder invasieve alternatieven, zoals embolisatie van de slagaders van de baarmoeder (uteriene arterie embolisatie (UAE)), worden daarom steeds vaker onderzocht.\n\nDit proefschrift richt zich enerzijds op de diagnostiek en bepaling van ernst van adenomyose met behulp van echografie, en anderzijds op UAE als alternatief voor hysterectomie bij symptomatische adenomyose. In deel 1 worden verschillende aspecten van de beeldvormende diagnostiek onderzocht. In deel 2 worden de klinische uitkomsten van UAE vergeleken met hysterectomie binnen de multicenter QUESTA-studie.\n\nDeel 1\nIn deel 1 van dit proefschrift staat de diagnostiek van adenomyose centraal, met een specifieke focus op echografische diagnostiek. Eerder adviseerde de MUSA-consensusgroep om specifieke directe en indirecte kenmerken van adenomyose systematisch te rapporteren. Directe kenmerken van adenomyose weerspiegelen de aanwezigheid van endometriumweefsel in het myometrium (ectopisch endometrium) en omvatten subendometriale \u2018lines and buds\u2019, hyperechogene eilandjes en myometrium (micro)cystes. Indirecte kenmerken zijn het gevolg van deze ectopische endometriumhaarden, zoals hypertrofie van het myometrium met een globulaire uterus of asymmetrisch myometrium, waaiervormige schaduwen veroorzaakt door cysten of bloedvaten, vascularisatie door de adenomyose laesie en een onderbroken of onregelmatige junctional zone (JZ). Deze uniforme terminologie heeft geleid tot consistenter onderzoek en verslaglegging, maar verschillende vragen bleven onbeantwoord. In deel 1 wordt daarom onderzocht waardoor adenomyose verschillende echografische verschijningsvormen kent, verklaard vanuit histologische bevindingen en pathogeenetische mechanismen, en wordt gezocht naar een objectievere methode om de ernst van adenomyose vast te stellen.\n\nHoofdstuk 2\nDe junctional zone is de binnenste laag van het myometrium net onder het endometrium (subendometriaal) en speelt vermoedelijk een essenti\u00eble rol in de pathogenese van adenomyose. Desondanks is de JZ geen eenduidig gedefinieerde structuur binnen de verschillende diagnostische disciplines. Histologisch bestaat de JZ uit geleidelijke veranderingen in cellulaire en vasculaire structuren richting het buitenste myometrium, terwijl bij beeldvorming een duidelijk onderscheid tussen de JZ en het buitenste myometrium wordt waargenomen. Bovendien wordt de JZ bij dezelfde pati\u00ebnt en in dezelfde cyclusfase vaak dikker afgebeeld op MRI dan op transvaginale echografie (TVS). Door deze inconsistenties tussen beeldvormende modaliteiten kan de dikte van de JZ niet langer worden beschouwd als gouden standaard voor de diagnose adenomyose. De nadruk zou daarom moeten liggen op kenmerken die zichtbaar zijn in alle modaliteiten: directe tekenen van ectopisch endometrium in de JZ of aanwijzingen voor door adenomyose aangedaan myometrium. Histologisch betreft dit endometriumklieren en stroma in het myometrium, zichtbaar op TVS als subendometriale \u2018lines and buds\u2019, hyperechogene eilanden, cysten in het myometrium of een onregelmatige JZ, en op MRI als hyperintense foci in de JZ of het myometrium. Een verdikte junctional zone kan worden verklaard door hypertrofie van gladde spiercellen als indirect teken van adenomyose.\n\nHoofdstuk 3\nNaast verschillen in het uiterlijk van de junctional zone worden bij adenomyose ook andere morfologische en weefselkarakteristieken beschreven. In dit hoofdstuk werden de verschillende pathogenetische mechanismen die betrokken zijn bij het ontstaan en de verergering van adenomyose onderzocht om beter te begrijpen welke processen verantwoordelijk zijn voor deze uiteenlopende weefselkarakteristieken. Er werd verondersteld dat echografische kenmerken vier histologische verschijningsvormen weerspiegelen: klierweefsel, microcystisch weefsel, vasculair weefsel en fibrotisch weefsel. Deze kunnen afzonderlijk of gelijktijdig voorkomen, in wisselende mate van dominantie, waardoor een continu spectrum van adenomyose fenotypes ontstaat. Mogelijk kunnen deze morfologische kenmerken beter zichtbaar worden gemaakt door aanvullende technieken naast conventionele 2D- en 3D-echografie toe te passen, zoals slow-flow imaging en elastografie. Slow-flow imaging visualiseert microvascularisatie, terwijl elastografie de stijfheid van weefsel ten opzichte van omliggend weefsel in beeld brengt. Tevens werd gespeculeerd over de relatie tussen echografische en histologische verschijningsvormen en hun mogelijke invloed op symptomen en respons op minimaal invasieve of medicamenteuze behandelingen. Het toevoegen van een adenomyose spectrum met fenotypekarakterisering met behulp van 2D- en 3D-echografie, kleurendoppler, MVFI en elastografie aan toekomstige uitbreidingen van het MUSA-classificatiesysteem kan toekomstig onderzoek verbeteren.\n\nHoofdstuk 4\nIn hoofdstuk 4 werd gezocht naar een objectievere methode om de ernst van adenomyose echografisch vast te leggen. De hypothese was dat niet het aantal aanwezige MUSA-kenmerken, maar de uitgebreidheid van het aangedane myometrium bepalend is voor de ernst van adenomyose en daarmee de relatie met symptomen zoals dysmenorroe, hevig menstrueelbloedverlies en subfertiliteit. De MUSA-consensusgroep stelde eerder voor om de uitgebreidheid van adenomyose subjectief in te delen in mild (<25% aangedaan), matig (25\u201350% aangedaan) of ernstig (>50% aangedaan). Deze indeling was echter niet gevalideerd en is sterk afhankelijk van de beoordelaar. Daarom werd gezocht naar een objectieve, semi-kwantitatieve methode die praktisch uitvoerbaar is en een goede interobserver betrouwbaarheid heeft. Dertig 3D volumes van uteri met adenomyose, gedefinieerd als aanwezigheid van ten minste \u00e9\u00e9n direct MUSA kenmerk, werden offline beoordeeld met behulp van het programma 5D-viewer. De beoordelaars waren geblindeerd voor de beoordeling tijdens het polikliniekbezoek. Zes mogelijke beoordelingsmethoden werden ontwikkeld, waarvan uiteindelijk drie praktisch uitvoerbaar bleken. Deze drie methoden werden vervolgens onderzocht op interobserver betrouwbaarheid. Uiteindelijk bleek \u00e9\u00e9n methode, waarbij twintig transversale coupes van het uterusvolume werden beoordeeld op aanwezigheid van directe MUSA-kenmerken, zowel praktisch toepasbaar als betrouwbaar: de \u2018XI VOCAL counting methode\u2019. Deze methode lijkt veelbelovend voor toekomstig onderzoek en kan bijdragen aan een objectievere en consistenter beoordeling van adenomyose.\n\nHoofdstuk 5\nDe in hoofdstuk 4 ontwikkelde \u2018XI VOCAL counting methode\u2019 werd in hoofdstuk 5 gebruikt om de relatie tussen echografische ernst van adenomyose en de ernst van symptomen te onderzoeken. In een prospectieve observationele studie werden 115 pati\u00ebnten ge\u00efncludeerd op de gynaecologische polikliniek van Amsterdam UMC. Dysmenorroe, chronische buik- of bekkenpijn en dyspareunie werden beoordeeld met een numerieke beoordelingsschaal (NRS: 0\u201310). Abnormaal menstrueel bloedverlies werd beoordeeld met een driepuntsschaal en gekwantificeerd met een pictorial blood assessment chart (PBAC). Zowel \u2018XI VOCAL counting\u2019 als de \u2018XI VOCAL-categorie\u00ebn\u2019 methodes correleerden positief met de ernst van menstrueel bloedverlies. Er werd echter geen correlatie gevonden tussen echografische ernst en dysmenorroe, chronische buik- of bekkenpijn of dyspareunie. De subjectieve beoordeling van de ernst van adenomyose door clinici tijdens het polikliniekbezoek correleerde wel significant met PBAC-scores en subjectief ervaren bloedverlies. De toepasbaarheid van \u2018XI VOCAL counting\u2019 in de klinische praktijk dient verder onderzocht te worden, onder andere in relatie tot de aangedane myometriumlaag en het uterusvolume. Daarnaast zijn de relatie tussen echografische ernst van adenomyose en subfertiliteit of miskramen interessante onderwerpen voor toekomstig onderzoek.\n\nDeel 2\nIn deel 2 van dit proefschrift werd uteriene arterie embolisatie (UAE) onderzocht als alternatief voor hysterectomie bij pati\u00ebnten met symptomatische adenomyose bij falen van conservatieve therapie. Dit onderzoek maakte deel uit van de multicenterstudie QUality of life after Embolization versus hySTerectomy for symptomatic Adenomyosis (QUESTA), een prospectieve cohortstudie met een follow-upduur van twee jaar, uitgevoerd in twaalf centra in Nederland. De studie startte oorspronkelijk als gerandomiseerde gecontroleerde trial.\n\nDoor UAE en hysterectomie met elkaar te vergelijken werd onderzocht of UAE een veelbelovend en minder invasief alternatief vormt voor hysterectomie, aangezien tot op heden vergelijkende gegevens ontbraken. In hoofdstuk 6 werd de primaire uitkomstmaat na \u00e9\u00e9n jaar follow-up beschreven: non-inferioriteit van UAE ten opzichte van hysterectomie op het gebied van gezondheidsgerelateerde kwaliteit van leven (HRQOL). HRQOL werd beoordeeld met de gevalideerde vragenlijsten SF-12 en WHO-QoL-Bref. De WHO-QoL-Bref omvat vier domeinen: fysiek, psychologisch, sociale relaties en omgeving. De SF-12 bestaat uit een mentale component score (MCS) en een fysieke component score (PCS). In hoofdstuk 7 werden de periprocedurele, korte termijn- en hersteluitkomsten besproken. In hoofdstuk 8 werden veranderingen in het myometrium op MRI zes maanden na UAE beschreven en werden beeldvormende baseline-kenmerken onderzocht als mogelijke voorspellers van deze veranderingen, met als doel de counseling van pati\u00ebnten op de polikliniek te verbeteren.\n\nHoofdstuk 6\nE\u00e9n jaar na de interventie kon - binnen de vooraf vastgestelde marges - zowel non-inferioriteit als inferioriteit van UAE ten opzichte van hysterectomie voor HRQOL niet worden aangetoond. UAE en hysterectomie resulteerden w\u00e9l in een significante verbetering van de kwaliteit van leven bij pati\u00ebnten met symptomatische adenomyose. De gecorrigeerde gemiddelden van de fysieke en mentale component scores van de SF-12 waren echter significant hoger in de hysterectomiegroep dan in de UAE-groep. De WHO-QoL-Bref liet vergelijkbare uitkomsten tussen beide groepen zien. Zowel UAE als hysterectomie verbeterden seksuele activiteit en pijnklachten, hoewel hysterectomie een sterker effect had op pijnreductie. UAE liet gunstigere resultaten zien ten aanzien van klachten van frequente mictie en incontinentie. Meer deelnemers waren tevreden na hysterectomie. Ondanks dat hysterectomie op verschillende domeinen beter presteerde, vormt UAE een goed alternatief voor pati\u00ebnten met adenomyose. UAE kan worden aangeboden aan pati\u00ebnten die geen radicale operatie wensen of hiervoor niet geschikt zijn, of aan pati\u00ebnten die hun uterus willen behouden. Voor pati\u00ebnten die een definitieve oplossing zoeken, blijft hysterectomie de voorkeursbehandeling.\n\nHoofdstuk 7\nDe peri-procedurele en korte termijn complicaties waren gering en vergelijkbaar tussen beide groepen. De opnameduur na hysterectomie was korter dan na UAE (1,40 versus 1,85 dagen; p=.008). Pati\u00ebnten na hysterectomie scoorden beter op de recovery index (39,4 versus 34,5; p=.009) en keken positiever terug op hun behandeling. UAE ging daarentegen gepaard met een kortere procedureduur (60 versus 109 minuten; p<.001) en minder bloedverlies (0 versus 50 ml; p<.001). Daarnaast keerden pati\u00ebnten na UAE sneller terug naar dagelijkse activiteiten en werk. Zo hervatten pati\u00ebnten hun werk gemiddeld na 17,5 dagen na UAE versus 43,8 dagen na hysterectomie (p<.001). Pati\u00ebnt-gecontroleerde intraveneuze analgesie bij UAE resulteerde in een kortere ziekenhuisopname dan epidurale analgesie en kan daarom worden overwogen als standaardbehandeling. Deze studie ondersteunt pati\u00ebnten met symptomatische adenomyose en hun behandelaars bij evidence-based besluitvorming tussen UAE en hysterectomie, waarbij de korte termijn uitkomsten kunnen worden meegewogen.\n\nHoofdstuk 8\nIn dit hoofdstuk werden de vijftig deelnemers die UAE ondergingen binnen de QUESTA-studie geanalyseerd. De studie richtte zich op MRI-karakteristieken na de behandeling, voorspellers van MRI-veranderingen en de relatie met verbetering van kwaliteit van leven. UAE resulteerde zes maanden na behandeling in een significante afname van het uterusvolume (van 254,2 cm\u00b3 naar 203,8 cm\u00b3; p<.001), het volume van adenomyose (van 75,4 cm\u00b3 naar 31,4 cm\u00b3; p<.001) en de dikte van de junctional zone (van 21 mm naar 17 mm; p=.0013). Een groter uterusvolume bij baseline hing positief samen met de mate van infarcering. Hyperintense foci op MRI, passend bij myometrium cysten, waren geassocieerd met een groter residueel uterusvolume en adenomyose volume na zes maanden. In deze relatief kleine onderzoekspopulatie kon geen verband worden aangetoond tussen infarceringsgraad, volumevermindering van uterus of adenomyose, afname van JZ-dikte en gezondheids-gerelateerde kwaliteit van leven na zes maanden follow-up.\n\nAlgemene conclusie\nDit proefschrift draagt bij aan verdere ontwikkeling van zowel de diagnostiek als de behandeling van adenomyose, met nadruk op meer gepersonaliseerde en evidence-based zorg. De resultaten laten zien dat adenomyose niet moet worden beschouwd als een binaire aandoening, maar eerder als een continu spectrum van verschillende fenotypes, gekenmerkt door uiteenlopende histologische en echografische kenmerken zoals glandulaire, microcystische, vasculaire en fibrotische patronen. Deze benadering kan bijdragen aan een betere classificatie van adenomyose, meer uniforme onderzoekspopulaties en uiteindelijk meer gerichte therapie\u00ebn. Daarnaast biedt de ontwikkeling van de \u2018XI VOCAL counting\u2019 methode een veelbelovende, semi-kwantitatieve manier om de ernst van adenomyose objectiever vast te leggen.\n\nOp therapeutisch gebied toont de QUESTA-studie aan dat zowel hysterectomie als uteriene arterie embolisatie (UAE) leiden tot een duidelijke verbetering van de kwaliteit van leven bij pati\u00ebnten met symptomatische adenomyose. Hoewel hysterectomie op verschillende domeinen betere resultaten liet zien, vormt UAE een waardevol minder invasief alternatief voor pati\u00ebnten die uterusbehoud wensen of chirurgie willen vermijden. Goede counseling, afgestemd op individuele voorkeuren en verwachtingen, blijft hierbij essentieel.\n\nToekomstig onderzoek zou zich moeten richten op verdere validatie van adenomyose fenotypes, toepassing van geavanceerde beeldvormingstechnieken, integratie van kunstmatige intelligentie en de ontwikkeling van nieuwe therapeutische strategie\u00ebn. Internationale samenwerking en grotere onderzoekspopulaties zijn daarbij noodzakelijk om verdere stappen te zetten richting gepersonaliseerde zorg voor pati\u00ebnten met adenomyose.","summary":"ENGLISH SUMMARY\n\nPart 1\nIn part one, we discuss the diagnosis of adenomyosis and focus on ultrasound diagnosis. Previously, the international Morphological Uterus Sonographic Assessment (MUSA) consensus group suggested reporting on specific direct and indirect features. Direct features of adenomyosis are signs of ectopic endometrial glands and stroma in the myometrium: subendometrial lines and buds, hyperechogenic islands, and intra-myometrial (micro-)cysts. Indirect features are the consequence of the ectopic endometrial glands: hypertrophic myometrium resulting in globular uterus or asymmetric myometrium; cysts or vessels reflecting echo sound beams resulting in fan-shaped shadowing, and intralesional vascularity, and interrupted or irregular junctional zone (JZ). These features resulted in more consistent research and reporting; however, some questions were still left unanswered. In part 1, we aim to unravel some of these questions by looking into the reason for different appearances on sonography (explained by histological findings or pathogenetic pathways) and by finding a more objective method to assess the severity of adenomyosis.\n\nChapter 2\nThe junctional zone is the inner subendometrial layer of the myometrium and is thought to play an essential role in the pathogenesis of adenomyosis. It is, however, not a well-defined entity across different diagnostic disciplines. In histology, the JZ exists out of gradual cellular and vascular changes towards the outer myometrium, while on imaging, a distinction between the JZ and the outer myometrium is found. Additionally, the JZ of the same patient during the same cycle phase is often visualized thicker on MRI than TVS. Given the inconsistencies among the imaging modalities, the thickness of the JZ cannot remain the gold standard for diagnosing adenomyosis. Therefore, the focus should be on markers for adenomyosis visible in all modalities, the so-called direct signs of ectopic endometrium in the JZ, or signs of myometrium affected by adenomyosis. On histology, these are spots of endometrial glands and stroma present in the myometrium, which are visualized on transvaginal sonography (TVS) as sub-endometrial lines and buds and hyperechogenic islands and cysts in the myometrium or as an irregular JZ or JZ interrupted by invaginated endometrium, and on MRI as bright high-intensity foci interrupting the JZ or myometrium. A thickened junctional zone might be explained by smooth muscle cell hypertrophy as an indirect sign of adenomyosis.\n\nChapter 3\nBesides the different appearance of the junctional zone in adenomyosis with different diagnostic modalities, other morphologies, and tissue characteristics are also reported in adenomyosis. We reviewed pathways reported in the pathogenesis and progression of adenomyosis to understand what mechanisms take place, possibly resulting in different tissue characteristics. We hypothesized that the sonographic features reflect four histological appearances: the presence of glandular, microcystic, vascular and fibrotic tissue. These can be present simultaneously or separately and with varying grades of predominance, creating a continuous spectrum of various adenomyosis phenotypes. These morphological characteristics might be better visualized by adding new modalities to conventional 2D- and 3D ultrasonography: slow-flow and elastography. Slow-flow visualizes microvascularity, while elastography visualizes the softness of tissue in comparison to the surrounding tissue. We speculated on the different appearances of ultrasound and histology and the potential impact on symptomatology and responsiveness to minimally invasive or medical therapy.\n\nIncorporating phenotype characterization- using 2D and 3D ultrasound, color Doppler, MVFI, and elastography- into future extensions of the MUSA reporting framework could enhance future research.\n\nChapter 4\nIn Chapter 4, we aimed to find a method to report on the sonographic severity of adenomyosis more objectively. We hypothesized that not the number of MUSA features present but the extent of the adenomyotic myometrium is important in assessing severity and correlation with symptomatology (e.g., dysmenorrhea, abnormal uterine bleeding, and subfertility). The MUSA consensus group suggested categorizing the extent of adenomyosis by an estimation into mild (<25% affected), moderate (25%-50% affected), or severe (>50% affected). This categorization was not validated and was very subjective. Therefore, we aimed to find a more objective way to semi-quantify the severity of adenomyosis, which was feasible in use and with good interobserver reliability. We collected 30 uterine volumes with adenomyosis (at least one direct MUSA feature present) and assessed the volumes offline, blinded for the evaluation at the outpatient clinic. We developed six potential offline methods using 5D-viewer. Of these six methods, only three were feasible regarding technique, time, and interpretation. These three methods were consecutively assessed for interobserver reliability. Eventually, one method, which included taking 20 transversal slices out of the uterine volume and evaluating each slice as affected (\u22651 direct MUSA feature present) was feasible and had good interobserver reliability: the XI VOCAL counting method. XI VOCAL counting appears promising in future research, making the assessment of adenomyosis more objective and consistent.\n\nChapter 5\nThe method developed and evaluated in Chapter 4, XI VOCAL counting, was further utilized in Chapter 5 to investigate the relationship between the sonographic severity and the severity of symptoms. An observational study was performed, and 115 patients were prospectively included at the Amsterdam UMC gynecological outpatient clinic. Dysmenorrhea, chronic abdominal\/pelvic pain, and dyspareunia were assessed with a numeric rate scale (NRS: 0-10), and abnormal menstrual bleeding with a three-point Likert scale and scored with a pictorial blood assessment chart (PBAC: 0- \u221e). A positive correlation was found between both XI-VOCAL counting and XI-VOCAL categories with the severity of menstrual blood loss. XI-VOCAL counting and XI-VOCAL categories did not correlate with the severity of dysmenorrhea, chronic abdominal\/pelvic pain, and dyspareunia. The subjective real-time severity assessed by clinicians at outpatient clinic visits showed a statistically significant correlation with PBAC and subjective blood loss. The applicability of XI VOCAL counting in the outpatient clinic should be further investigated, including assessing the affected layer(s), and uterine volume. Also, the relation between sonographic severity and subfertility and miscarriages is interesting for further research.\n\nPart 2\nIn part 2, we investigated uterine artery embolization as an alternative to hysterectomy for patients with symptomatic adenomyosis when conservative therapy fails. This study was part of the multicenter QUality of life after Embolization versus hySTerectomy for symptomatic Adenomyosis (QUESTA) study, a two-year prospective cohort study in 12 centers in the Netherlands that initially started as a randomized controlled trial.\n\nBy comparing UAE and hysterectomy, we aimed to answer whether UAE is a promising and less-invasive alternative for hysterectomy because comparative data has been lacking up to now. In Chapter 6, we reported on the primary objective after one year: non-inferiority of embolization versus hysterectomy regarding health-related quality of life (HRQOL). HRQOL is assessed using validated questionnaires SF-12 and WHO-QoL-Bref. The WHO-QoL-Brev exists out of four domains: physical, psychological, social relations, and environment. The SF-12 is divided into a mental component summary score (MCS) and a physical component summary score (PCS). In chapter 7, the peri-procedural, short-term, and recovery outcomes were discussed. In Chapter 8, we described the myometrial change on MRI six months after UAE and explored baseline imaging variable as potential predictors for this change, which might be used in counseling patients at the outpatient clinic.\n\nChapter 6\nAfter a one-year follow-up, neither non-inferiority nor inferiority of UAE versus hysterectomy for HRQOL in symptomatic adenomyosis could be established within the predefined margins. Both UAE and hysterectomy significantly improved HRQOL for patients with symptomatic adenomyosis. Nevertheless, adjusted means of the SF-12 physical and mental component summary score (PCS and MCS) at one year were statistically significantly higher in the hysterectomy group compared to the UAE group; the other HRQOL questionnaire (WHO-QOLBrev) resulted in comparable outcomes between groups. Both UAE and hysterectomy improved sexual activity and pain, although hysterectomy had a more significant effect on pain reduction. UAE performed better regarding symptoms of urinary frequency and incontinence. More participants were satisfied after hysterectomy. Even though hysterectomy performs better in several domains, UAE is a good alternative in adenomyosis patients. It can be offered to patients who do not want or are not suitable to undergo radical surgery or who want to preserve their uterus. For patients seeking a definitive solution, hysterectomy remains the treatment of choice.\n\nChapter 7\nThe periprocedural and short-term complication rates were minor and comparable between groups. Hospital stay after hysterectomy was shorter than after UAE (1.40 versus 1.85 days; p=.008); hysterectomy patients scored better on the recovery index (hysterectomy: 39.4 versus UAE: 34.5, p=.009), and reflected more positively on their perceived procedure. UAE favored faster procedure time (60 minutes versus 109 minutes, p<.001) and less blood loss (0 ml versus 50 ml, p<.001), and patients returned faster to their daily activities and work. For example, patients returned to work 17.5 days after UAE and 43.8 days after hysterectomy (p<.001). Patient-controlled intravenous analgesia for UAE resulted in shorter hospital stay than patient-controlled epidural analgesia and may be considered standard clinical practice. This study assists patients with symptomatic adenomyosis and their doctors in evidence-based decision-making regarding UAE and hysterectomy by weighing short-term outcomes.\n\nChapter 8\nThis study analyzed the 50 participants undergoing UAE as part of the QUESTA study. It focused on post-procedural MR imaging characteristics, predictors for MRI change, and the relation with improvement in quality of life. UAE resulted in a significant decrease in uterine volume at six months compared to baseline (from 254.2 cm3 to 203.8 cm3, p<.001), adenomyosis volume (from 75.4cm3 to 31.4 cm3, p<.001), and JZ thickness (from 21mm to 17mm, p=.0013). Uterine volume at baseline positively impacted the rate of infarction (exp(\u03b2) = 1.0074, 95% CI 1.0001 - 1.0148). High-intensity foci (representing myometrial cysts) were related to a higher residual uterine volume (exp(\u03b2) 1.351, 95% CI: 0.947 \u2013 1.912) and a higher residual adenomyosis volume (exp(\u03b2) 1.885, 95% CI: 0.967 \u2013 3.572) at six-months. In our small sample, a correlation between infarction rate, reduction of uterine volume, adenomyosis volume or JZ thickness, and health-related quality of life at 6 months follow-up could not be demonstrated.\n\nGeneral conclusion\nThis thesis contributes to the further development of both the diagnosis and treatment of adenomyosis, with an emphasis on more personalized and evidence-based care. The results show that adenomyosis should not be considered a binary condition, but rather a continuous spectrum of different phenotypes, characterized by diverse histological and ultrasound features such as glandular, microcystic, vascular, and fibrotic patterns. This approach can contribute to a better classification of adenomyosis, more uniform study populations, and ultimately more targeted therapies. Additionally, the development of the 'XI VOCAL counting' method offers a promising, semi-quantitative way to more objectively determine the severity of adenomyosis.\n\nIn the therapeutic field, the QUESTA study shows that both hysterectomy and uterine artery embolization (UAE) lead to a clear improvement in the quality of life in patients with symptomatic adenomyosis. Although hysterectomy showed better results in several domains, UAE is a valuable less-invasive alternative for patients who wish to preserve their uterus or want to avoid surgery. Good counseling, tailored to individual preferences and expectations, remains essential here.\n\nFuture research should focus on further validation of adenomyosis phenotypes, application of advanced imaging techniques, integration of artificial intelligence, and the development of new therapeutic strategies.
