De behandeling met vaste apparatuur brengt echter ook risico\u2019s op ongewenste bijwerkingen met zich mee, waarvan vroege cari\u00ebslaesies in de vorm van white spot laesies (WSL\u2019s) en externe apicale wortelresorptie (EARR) de belangrijkste zijn. White spot laesies zijn gedemineraliseerde glazuurlaesies die zich doorgaans ontwikkelen rondom orthodontische brackets, banden en draden. Ze ontstaan wanneer plaque zich ophoopt als gevolg van onvoldoende mondhygi\u00ebne en een cariogeen voedingspatroon, waarna zuurvormende microbi\u00eble activiteit leidt tot demineralisatie van het glazuur. White spot laesies kunnen al binnen enkele weken na het plaatsen van de orthodontische apparatuur ontstaan en komen frequent voor. Na afloop van de behandeling zijn ze vaak nog zichtbaar als esthetische \u2018littekens\u2019 aan het glazuuroppervlak. Externe apicale wortelresorptie betreft een verkorting van de tandwortels, veroorzaakt door resorptieprocessen in het parodontale ligament en op het worteloppervlak onder invloed van orthodontische krachten. Hoewel deze resorptie in de meeste gevallen mild en klinisch weinig relevant is, kunnen ernstige vormen de langetermijnstabiliteit van gebitselementen in gevaar brengen. Beide complicaties worden beschouwd als grotendeels te voorkomen of klinisch beheersbaar, mits evidence-based preventieve strategie\u00ebn consequent worden toegepast.\n\nDe afgelopen jaren heeft de Nederlandse Vereniging van Orthodontisten (NVvO) verschillende klinische praktijkrichtlijnen ontwikkeld, waaronder voor de preventie van WSL\u2019s (uitgebracht in 2022) en voor de diagnostiek en behandeling van EARR (uitgebracht in 2018). De toepassing van deze richtlijnen in de dagelijkse praktijk is echter onbekend. Het onderzoek dat in dit proefschrift wordt beschreven had tot doel de daadwerkelijke toepassing van de preventieve strategie\u00ebn voor WSL\u2019s en EARR beschreven in richtlijnen te evalueren, met de nadruk op pati\u00ebntgedrag, preventieve interventies en de professionele implementatie van richtlijnen.\n\nIn hoofdstuk 1 wordt een algemene inleiding gegeven, waarin de hoge prevalentie van malocclusie en de wereldwijd toenemende vraag naar orthodontische behandeling worden beschreven. Tegen deze achtergrond is orthodontische behandeling uitgegroeid tot een gangbaar onderdeel van de mondzorg voor adolescenten. De voordelen lijken daarbij vooral psychosociaal van aard, zoals een verbeterde esthetiek en een groter zelfvertrouwen, terwijl het bewijs voor langetermijnverbeteringen in functie en mondgezondheid minder consistent is. Het hoofdstuk introduceert verder de twee belangrijkste bijwerkingen die gepaard gaan met de behandeling met vaste apparatuur, WSL\u2019s en EARR. White spot laesies zijn grotendeels gedragsafhankelijk en te voorkomen door adequate mondhygi\u00ebne en dieetcontrole, terwijl EARR biologisch gemedieerd is, vooralsnog niet voorkomen kan worden en zorgvuldige klinische monitoring en behandeling vereist.\n\nIn hoofdstuk 2 wordt een narratief literatuuroverzicht gepresenteerd waarin klinische vraagstellingen rond WSL\u2019s en EARR, zoals deze in de dagelijkse orthodontische praktijk voorkomen, worden behandeld. Uit dit literatuuroverzicht blijkt dat de preventie van EARR complex blijft, waarbij toenemend bewijs suggereert dat genetische predispositie een doorslaggevende rol speelt in individuele vatbaarheid. Er bestaan (nog) geen effectieve klinische strategie\u00ebn om deze genetische risicofactoren te be\u00efnvloeden. Het ontstaan van WSL\u2019s is daarentegen grotendeels te voorkomen bij strikte naleving van preventieve maatregelen. Hoewel het naleven van deze maatregelen bij iedere orthodontische pati\u00ebnt wordt gestimuleerd, blijft de daadwerkelijke naleving vaak suboptimaal. Het richtlijnontwikkelingsproces bracht bovendien belangrijke kennislacunes aan het licht. Voor EARR ontbreekt betrouwbare kennis over de pati\u00ebntgebonden risicofactoren en klinisch voorspellende indicatoren. Voor WSL\u2019s bestaan lacunes in het voorspellen van het ontstaan en beloop van laesies, met name op de lange termijn. Verder is de kosteneffectiviteit van preventieve interventies onderbelicht en worden pati\u00ebntgerichte uitkomsten zelden gerapporteerd. Ook ontbreken specifieke aanbevelingen voor alignertherapie.\n\nIn hoofdstuk 3 wordt een cross-sectioneel onderzoek beschreven naar de naleving van preventieve aanbevelingen ter voorkoming van WSL\u2019s bij adolescenten met vaste orthodontische apparatuur. In het onderzoek werden 393 adolescenten van 12 tot en met 17 jaar ge\u00efncludeerd die werden behandeld met vaste apparatuur in tien verschillende orthodontiepraktijken in Noordoost-Nederland. De naleving van de WSL-preventierichtlijn werd ge\u00ebvalueerd aan de hand van de opvolging van zes aanbevelingen: tweemaal daags tandenpoetsen, twee minuten poetsen, gebruik van fluoridetandpasta, interdentaal reinigen, jaarlijkse gebitscontrole en het beperken van eet- en drinkmomenten tot maximaal zeven per dag. De mediane nalevingsscore bedroeg vijf van de zes aanbevelingen, terwijl slechts 22,6% van de adolescenten alle aanbevelingen volledig opvolgde. Jongens en adolescenten op een hoger opleidingsniveau lieten significant lagere nalevingsscores zien; ook oudere adolescenten scoorden lager. Hoewel 88,5% aangaf ten minste tweemaal daags te poetsen en 85,5% in het voorafgaande jaar een tandarts of mondhygi\u00ebnist had bezocht, wist minder dan de helft zeker dat de gebruikte tandpasta fluoride bevatte, terwijl iets meer dan de helft ragers gebruikte (52,7%). Deze bevindingen onderstrepen dat cari\u00ebspreventief gedrag bij een aanzienlijk deel van de orthodontisch behandelde pati\u00ebnten suboptimaal blijft.\n\nHoofdstuk 4 beschrijft een secundaire analyse van het in hoofdstuk 3 beschreven onderzoek, waarin werd onderzocht of bewustzijn van WSL\u2019s en de ervaren hinder daarvan samenhangen met naleving van richtlijnen. Achtendertig procent van de adolescenten had voorafgaand aan het onderzoek WSL\u2019s gezien. Deze groep rapporteerde significant meer ervaren hinder van WSL\u2019s dan adolescenten die geen afbeelding hadden gezien van WSL\u2019s voordat zij deelnamen aan dit onderzoek (mediaan 13 versus 12 op een schaal van 3 tot 15), maar de nalevingsscores verschilden niet tussen beide groepen. Er werd een zwakke, maar statistisch significante correlatie gevonden tussen ervaren hinder en naleving van de aanbevelingen. Bewustzijn leidde dus wel tot meer bezorgdheid, maar niet tot beter preventief gedrag, hetgeen de kloof tussen kennis en gedrag in deze leeftijdsgroep benadrukt.\n\nIn hoofdstuk 5 wordt een gerandomiseerde, dubbelblinde, gecontroleerde klinische studie beschreven naar het effect van een plaque-identificerende tandpasta op plaquecontrole bij adolescenten met vaste orthodontische apparatuur. De plaquekleuring geeft visuele feedback tijdens het poetsen en het effect hiervan werd onderzocht bij adolescenten die met vaste apparatuur behandeld werden. In totaal werden 77 adolescenten gedurende zes weken gevolgd, ingedeeld in twee groepen. De ene groep poetste met een plaque-identificerende tandpasta terwijl de andere groep een visueel vergelijkbare controletandpasta gebruikte. De primaire uitkomstmaat, plaque-reductie gemeten met quantitative light-induced fluorescence (QLF), liet geen statistisch significant verschil zien tussen beide groepen. In beide groepen nam de hoeveelheid plaque in de tijd enigszins af, waarschijnlijk als gevolg van gestandaardiseerde mondhygi\u00ebne-instructies bij aanvang. Subgroepanalyses toonden geen effect van poetsgewoonten of sociaaleconomische status. Secundaire uitkomsten lieten zien dat gebruikers van de plaque-identificerende tandpasta deze als effectiever ervoeren, maar ook enige hinder ondervonden van de kleur. De therapietrouw was in beide groepen hoog en er werden geen bijwerkingen gerapporteerd. Deze resultaten suggereren dat visuele bewustwording van plaque op zichzelf onvoldoende is om objectief meetbare verbeteringen in mondhygi\u00ebne te realiseren binnen het onderzochte tijdsbestek.\n\nIn hoofdstuk 6 wordt een landelijk vragenlijstonderzoek beschreven naar de implementatie van de Nederlandse klinische praktijkrichtlijn voor EARR (uitgebracht in 2018) onder orthodontisten die lid zijn van de NVvO. Alle 275 praktiserende orthodontisten in Nederland kregen een digitale vragenlijst toegestuurd; 132 ingevulde vragenlijsten werden geanalyseerd (respons 48%). Vrijwel alle respondenten (98,5%) rapporteerden veranderingen in hun klinische handelen na introductie van de richtlijn. De mediane totale nalevingsscore steeg significant van 74 v\u00f3\u00f3r introductie naar 83 drie jaar later (maximumscore van 91). De grootste verbeteringen betroffen het handelen na vastgestelde EARR, waarbij orthodontisten vaker de behandeling (tijdelijk) onderbraken, orthodontische krachtgroottes reduceerden en pati\u00ebnten informeerden over de mogelijke consequenties van wortelresorptie. Demografische factoren zoals geslacht, werkervaring, land van opleiding en de praktijksetting hadden geen significante invloed op de implementatie. Deze resultaten tonen aan dat richtlijnen effectief kunnen bijdragen aan gestandaardiseerde, evidence-based zorg.\n\nIn hoofdstuk 7 (algemene discussie) worden de bevindingen van dit proefschrift in samenhang en vanuit zowel pati\u00ebnt- als zorgverlenersperspectief ge\u00efnterpreteerd. Adolescenten vormen een duidelijke risicogroep en voldeden slechts in beperkte mate aan de preventieve aanbevelingen ter voorkoming van WSL\u2019s. Bewustzijn van WSL\u2019s verhoogde de ervaren hinder, maar leidde niet tot betere naleving van preventieve maatregelen, wat wijst op de noodzaak van effectievere en gecombineerde motivatiestrategie\u00ebn. De onderzochte plaque-identificerende tandpasta vergrootte de bewustwording, maar resulteerde niet in significante plaque-reductie. Visuele feedback alleen blijkt dus onvoldoende zonder aanvullende educatieve of gedragsmatige ondersteuning. Orthodontisten bleken daarentegen bereid en in staat te zijn om richtlijnen te implementeren, zoals ge\u00efllustreerd door de brede toepassing van de praktijkrichtlijn voor EARR. Demografische factoren hadden hierbij geen significante invloed op de mate van implementatie. Het verschil tussen pati\u00ebnt- en zorgverlenersgedrag benadrukt dat hoewel professioneel handelen relatief snel kan worden gestandaardiseerd, pati\u00ebntgedrag de belangrijkste belemmering blijft voor het behoud van mondgezondheid tijdens orthodontische behandeling.\n\nSterke punten van dit proefschrift zijn: het grootschalige vragenlijstonderzoek, de objectieve QLF-metingen en de inzet van een regionaal onderzoeksnetwerk. Beperkingen betreffen het gebruik van zelfgerapporteerde gegevens, het cross-sectionele karakter van enkele studies en de relatief korte interventieduur. Toekomstig onderzoek zou zich moeten richten op longitudinale onderzoeken, digitale ondersteuning van de naleving van pati\u00ebntrichtlijnen en multifactori\u00eble interventies die klinische, educatieve en gedragsmatige componenten combineren.\n\nConcluderend blijven orthodontische pati\u00ebnten, ondanks de beschikbaarheid van richtlijnen, risico lopen op bijwerkingen zoals WSL\u2019s en EARR. Orthodontisten blijken in staat en bereid evidence-based maatregelen met betrekking tot EARR toe te passen, terwijl adolescenten moeite hebben om preventief gedrag ter voorkoming van WSL\u2019s consequent vol te houden. Het overbruggen van deze discrepantie, namelijk tussen de kennis van preventieve adviezen en het gedrag van pati\u00ebnten, vormt de primaire uitdaging voor WSL-preventie in de orthodontie. Gecombineerde strategie\u00ebn gericht op educatie, motivatie en het ondersteunen van therapietrouw van pati\u00ebnten, in combinatie met het blijven ontwikkelen en opvolgen van professionele richtlijnen door orthodontisten, zijn essentieel om optimale behandelresultaten te behalen met behoud van een goede mondgezondheid.","summary":"Orthodontic treatment with fixed appliances effectively corrects malocclusions and enhances oral health-related quality of life. However, this type of treatment also carries risks of adverse effects, notably white spot lesions (WSLs), which are non-cavitated carious lesions, and external apical root resorption (EARR). White spot lesions are demineralised enamel areas that typically develop around orthodontic brackets, bands and wires. They result from plaque accumulation due to inadequate oral hygiene and a cariogenic diet, whereby acid-producing microbial activity leads to enamel demineralisation. They can develop within weeks after appliance placement, are very common, and may remain as aesthetically displeasing \u201cscars\u201d on the teeth for many years after treatment. Regarding root resorption, this involves the shortening of tooth roots due to resorption processes in the periodontal ligament and on the root surfaces under orthodontic forces. Although usually mild and clinically insignificant, severe cases can threaten long-term tooth stability. Both complications are considered preventable or manageable if evidence-based preventive strategies are consistently applied.\n\nIn recent years, the Dutch Association of Orthodontists (Nederlandse Vereniging van Orthodontisten, NVvO) has established clinical practice guidelines (CPGs) for the prevention of WSLs (2022) and for the diagnosis and management of EARR (2018). However, translating these guidelines into daily orthodontic practice remains challenging. The research described in this thesis aimed to investigate how preventive strategies for WSLs and EARR are implemented in real-world settings, with a focus on patient behaviour, preventive interventions, and professional implementation of guidelines.\n\nIn Chapter 1, a general introduction is provided outlining the prevalence of malocclusion and the increasing demand for orthodontic treatment worldwide. Orthodontic treatment has become a common component of oral healthcare for adolescents, with benefits that are primarily psychosocial in nature, including improved dental aesthetics and increased self-confidence. Evidence for long-term improvements in function and oral health remains less consistent. The chapter further introduces the two main adverse effects associated with fixed appliance treatment, WSLs and EARR. White spot lesions are largely behaviour-dependent and preventable through adequate oral hygiene and dietary control, whereas EARR is biologically mediated, cannot be prevented, and requires careful clinical monitoring and management.\n\nIn Chapter 2, a narrative review examines clinical questions that orthodontists encounter in daily practice regarding WSLs and EARR. The review indicates that the prevention of EARR remains complex, with increasing evidence suggesting that genetic predisposition influences susceptibility. Currently, no effective clinical strategies are available to modify these genetic risk factors. Conversely, the development of WSLs is largely preventable, and strict preventive protocols should be applied to all orthodontic patients. However, patient adherence to these measures often remains inadequate. The guideline development process also highlighted significant knowledge gaps for both conditions. For EARR, reliable data on risk factors and predictive indicators are missing. At the same time, for WSLs, there are gaps in long-term risk prediction, the cost-effectiveness of preventive measures, patient-reported outcomes, and treatment-specific advice for clear aligner therapy. Addressing these gaps through high-quality research is essential for further development and updating of CPGs.\n\nIn Chapter 3, adolescents\u2019 adherence to recommendations for the prevention of WSLs was examined. A large cross-sectional survey was conducted amongst 393 patients aged 12 to 17 years undergoing fixed-appliance treatment in ten orthodontic practices in the northeast of the Netherlands. Adherence to six national preventive recommendations was assessed: brushing twice daily, brushing for 2 minutes, use of fluoride toothpaste, interdental cleaning, annual dental check-ups, and limiting food and drink intake to 7 times per day. A median adherence score of 5 out of 6 behaviours was observed, but only 22.6% of patients fully adhered to all recommendations. Males and participants with higher levels of education showed significantly lower adherence scores; older adolescents also had lower adherence scores. Although 88.5% reported brushing their teeth at least twice daily and 85.5% had visited a dentist or dental hygienist in the preceding year, fewer than half were certain that their toothpaste contained fluoride, whilst just over half used interdental brushes (52.7%). These findings highlight that daily caries-preventive behaviour remains suboptimal amongst a substantial proportion of adolescents undergoing orthodontic treatment.\n\nIn Chapter 4, the researchers performed a secondary analysis of the same survey data to determine whether awareness of WSLs and perceived annoyance influenced CPG adherence. Thirty-eight percent of adolescents had seen WSLs before taking part in the survey. These individuals reported significantly higher annoyance scores than those unaware of WSLs (median 13 versus 12 on a 3-15 scale), but adherence scores did not differ between the groups. A weak yet statistically significant correlation was found between annoyance and adherence. Therefore, increased awareness of WSLs heightened concern but did not lead to improved preventive behaviours, highlighting the gap between knowledge and action in this age group.\n\nIn Chapter 5, the researchers evaluated the effect of a relatively under-researched preventive intervention. A plaque-identifying toothpaste designed to provide visual feedback during brushing was tested amongst adolescents undergoing fixed appliance treatment. In a randomised, double-blind, controlled clinical trial, 77 adolescents were assigned to either the plaque-identifying toothpaste or a visually similar control toothpaste for six weeks. The primary outcome, plaque reduction measured objectively with quantitative light-induced fluorescence (QLF), showed no statistically significant difference between the two groups. Both groups exhibited some reduction in plaque over time, likely reflecting the effect of standardised oral hygiene instructions given at baseline. Subgroup analyses revealed no influence of brushing habits or socioeconomic status. Secondary outcomes indicated that patients using the plaque-identifying toothpaste perceived it as more effective at removing plaque but also reported some difficulty with its colour. Compliance was high in both groups, and no adverse effects were reported. These findings suggest that although plaque-identifying toothpaste can enhance subjective awareness of plaque, awareness alone is insufficient to improve objective oral hygiene outcomes in adolescents within the studied timeframe.\n\nIn Chapter 6, the results of a nationwide survey are reported, which examined the implementation of the Dutch CPG for EARR, amongst orthodontists three years after its introduction in 2018. A digital survey was sent to all 275 practising orthodontists in the Netherlands, and 132 responses were analysed (response rate 48%). Almost all respondents (98.5%) reported changes in clinical practice following the introduction of the guideline. The median total adherence score increased significantly from 74 at baseline to 83 at three-year follow-up (maximum of 91 points). The most notable improvements occurred in the management of EARR once detected during treatment. Orthodontists more frequently paused treatment, reduced the applied force level, and informed patients about potential consequences. Demographic factors such as sex, years of experience, training location, and practice type did not significantly influence adherence. These results show that guideline dissemination through professional networks can effectively contribute to more standardised and evidence-based clinical practice.\n\nIn Chapter 7, the findings of this thesis are synthesised to provide a comprehensive overview of preventive and interceptive measures and CPG adherence during orthodontic treatment, from both patient and clinician perspectives. In the patient group, adolescents showed limited adherence to preventive CPGs, with certain subgroups at higher risk of suboptimal behaviours. Awareness of WSLs increased perceived annoyance but did not enhance adherence, emphasising the need for more effective motivational strategies. On the intervention front, plaque-identifying toothpaste raised awareness but did not lead to a significant reduction in plaque, indicating that education or behavioural reinforcement is needed alongside visual feedback. Amongst providers, orthodontists demonstrated a strong willingness to follow guidelines, as evidenced by the widespread reported adoption of the CPG for EARR. The difference between patient and provider behaviours suggests that while professional practice can be quickly standardised, patient behaviours remain the main obstacle to successfully upholding oral health.\n\nThe strengths of this thesis include the large-scale survey study, objective QLF measurements, and the establishment of a regional practice-based research network, which facilitated recruitment and ensured that the results reflect daily clinical practice. Limitations include reliance on self-reported adherence data, which may be subject to response bias; the cross-sectional design of the adherence and awareness studies, which prevents causal inference; and the relatively short duration and single-centre setting of the intervention trial. Subsequent research should focus on longitudinal designs, digital tools to reinforce adherence, and multifactorial interventions combining clinical, educational, and behavioural strategies.\n\nIn conclusion, orthodontic patients remain at risk of adverse effects, such as WSLs and EARR, despite the availability of effective guidelines for their management. Orthodontists appear capable and willing to implement evidence-based preventive measures for EARR. On the other hand, adolescents find it difficult to consistently adhere to the recommended measures for WSL prevention. Bridging the gap between knowledge and action appears to be the main challenge in orthodontic patients regarding prevention and management.
