Het gebruik van deze materialen zal in de toekomst verder toenemen door vergrijzing, obesitas en de wereldwijde toegankelijkheid van deze operaties. Hoewel deze medische hulpmiddelen onmiskenbaar bijdragen aan het verminderen van pijn, het verbeteren van beweeglijkheid en het verhogen van de kwaliteit van leven, treedt bij ongeveer 2 \u2013 31% van de pati\u00ebnten na deze operaties een infectie op. Infecties rondom metalen kunstgewrichten en botbreuk-gerelateerde metalen materialen worden voornamelijk veroorzaakt door Staphylococcus-bacteri\u00ebn, zoals Staphylococcus aureus.\n\nDe huidige behandeling voor acute infecties bestaat uit het grondig schoonmaken van het implantaat en het operatief verwijderen van ge\u00efnfecteerd en dood weefsel daaromheen, gevolgd door minimaal 6 weken behandeling met antibiotica. Deze procedure, bekend als DAIR, is in ongeveer 70% van de gevallen succesvol. Wanneer DAIR niet effectief is, kan worden besloten de procedure te herhalen of het ge\u00efnfecteerde implantaat te vervangen door een nieuw implantaat. Helaas zijn deze laatstgenoemde operaties in circa 14% van de gevallen niet succesvol, waardoor pati\u00ebnten langdurig \u2013 soms jarenlang \u2013 antibiotica moeten gebruiken, met alle bijwerkingen van dien. Bovendien zijn vaak meerdere operaties noodzakelijk en is in zeldzame gevallen zelfs amputatie nodig om de infectie te bestrijden. Deze aanvullende ingrepen gaan gepaard met verminderde beweeglijkheid evenals meer pijn en psychische klachten, wat leidt tot een aanzienlijke afname van de kwaliteit van leven.\n\nImplantaat-geassocieerde infecties zijn moeilijk te behandelen doordat bacteri\u00ebn zoals Staphylococcus aureus zich aan het metalen oppervlak kunnen hechten. Op het implantaat vermenigvuldigen zij zich, waarbij ze een beschermende laag van suikers, eiwitten, vetten en DNA om zich heen vormen (hoofdstuk 1). Deze zogenoemde biofilm vormt een compacte structuur die zowel voor antibiotica als voor cellen en factoren van het afweersysteem slecht doordringbaar is. Daarnaast zijn biofilms slecht zichtbaar en bevinden zij zich vaak op plaatsen die voor de chirurg lastig bereikbaar zijn, wat het reinigen van het implantaat bemoeilijkt. Binnen biofilms ontstaan bacteri\u00eble subpopulaties met uiteenlopende gevoeligheden voor antibiotica. Sommige bacteri\u00ebn kunnen in een soort \u2018winterslaap\u2019 gaan, waardoor ze tijdelijk minder gevoelig zijn voor antibiotica. Tevens kunnen bacteri\u00ebn \u2013 spontaan of onder invloed van bijvoorbeeld antibiotica \u2013 uit de biofilm vrijkomen en op een andere plek in het lichaam een nieuwe infectie veroorzaken.\n\nHet falen van de antibiotica-behandeling als gevolg van de bovengenoemde processen tijdens de biofilm vorming draagt bij aan de ontwikkeling van ongevoeligheid voor antibiotica in bacteri\u00ebn (antibioticaresistentie). Het is daarom noodzakelijk dat alternatieve strategie\u00ebn worden ontwikkeld om deze infecties te bestrijden en de ontwikkeling van antibioticaresistentie te beperken. De studies in dit proefschrift richten zich op drie alternatieven voor antibiotica en hun combinaties in relatie tot biofilms: bacteriofagen, virussen die bacteri\u00ebn doden (hoofdstukken 2, 3, 4 en 6), verhitting van de metalen onderdelen van het implantaat met behulp van inductie (inductieverhitting; hoofdstukken 5 en 6) en antimicrobi\u00eble peptiden (hoofdstukken 5 en 6). \n\nAntimicrobi\u00eble peptiden (AMPs) zijn kleine moleculen die in de natuur (onder andere in het menselijke immuunsysteem) voorkomen en actief zijn tegen diverse ziekteverwekkers. AMPs kunnen de celwand van de bacterie kapotmaken, waardoor de bacterie als een ballon leegloopt en doodgaat. Op basis van natuurlijke AMPs zijn synthetische varianten ontwikkeld. Een daarvan is SAAP-148, dat zeer effectief is tegen onder andere Staphylococcus aureus.\n\nBacteriofagen zijn natuurlijke vijanden van bacteri\u00ebn en zijn zeer specifiek voor bepaalde bacteriesoorten en -stammen. Zodra deze virussen specifieke structuren op het oppervlak van de bacterie (celwand) herkennen, injecteren zij hun genetisch materiaal in de bacterie. Vervolgens nemen de bacteriofagen interne systemen van de bacterie over, waardoor de bacterie nieuwe bacteriofagen gaat produceren. In de laatste fase van deze virale cyclus barst de bacterie open, waardoor alle nieuw gevormde bacteriofagen vrijkomen en de bacterie doodgaat. Deze nakomelingen kunnen vervolgens andere gevoelige bacteri\u00ebn infecteren en doden.\n\nMomenteel wordt bacteriofaagtherapie in de westerse wereld uitsluitend als laatste redmiddel toegepast bij moeilijk behandelbare infecties. Mede hierdoor is er nog weinig bekend over de effectiviteit van bacteriofaagtherapie tegen bacteri\u00ebn in de verschillende fasen van de biofilm. Om dit te onderzoeken zijn de verschillende fasen van de biofilm in het laboratorium nagebootst op kleine metalen schijfjes, gemaakt van hetzelfde materiaal als klinische implantaten. Hoofdstuk 2 beschrijft het onderzoek naar de effecten van bacteriofaag ISP, die specifiek gericht is op Staphylococcus aureus. Hoewel ISP zeer effectief is tegen bacteri\u00ebn in suspensie, neemt de effectiviteit af naarmate de bacteri\u00ebn zich aan het metalen oppervlak hechten en een biofilm vormen. Desondanks blijkt ISP in staat de biofilm binnen te dringen en interacties aan te gaan met de aanwezige bacteri\u00ebn. De hoeveelheid bacteriofagen in de biofilm is echter aanzienlijk lager dan aan welke de biofilm initieel is blootgesteld. Daarnaast is ISP minder effectief tegen bacteri\u00ebn in \u2018slaaptoestand\u2019, evenals tegen bacteri\u00ebn die spontaan uit biofilms vrijkomen. Bij deze bacteri\u00ebn is de ongevoeligheid voor ISP slechts tijdelijk.\n\nHet onderzoek dat in hoofdstuk 3 is beschreven, had als doel de overlevingsmechanismen te identificeren van bacteri\u00ebn die spontaan uit biofilms vrijkomen en worden blootgesteld aan bacteriofagen. De resultaten laten zien dat een subpopulatie van deze bacteri\u00ebn tijdelijk ongevoelig is voor de bacteriofagen ISP en Sb-1 bij hoge doseringen. In het onderzoek werden alle eiwitten van de gevoelige en de ongevoelige bacteri\u00ebn met elkaar vergeleken. Op basis hiervan zijn twee mogelijke overlevingsmechanismen ge\u00efdentificeerd: een versterkte celwand en een verlaagde stofwisseling. Ondanks de versterkte celwand kunnen bacteriofagen hun genetisch materiaal toch in deze bacteri\u00ebn injecteren. Het is mogelijk dat bacteriofagen niet in staat zijn de celwand van de bacterie in de laatste fase te openen. Een verlaagd metabolisme zorgt er mogelijk ook voor dat er minder bacteriofagen worden geproduceerd, wat eveneens bijdraagt aan het overleven van de bacteri\u00ebn als groep.\n\nIn de praktijk wordt bacteriofaagtherapie vaak gecombineerd met antibiotica, waarbij de keuze van deze combinaties afhangt van de gevoeligheid van de bacterie voor beide behandelingen. Bacteriofagen en antibiotica kunnen elkaars werking versterken of juist verzwakken, waardoor het selecteren van de juiste combinatie cruciaal is. Hiervoor moet de effectiviteit van bacteriofaag-antibioticacombinaties worden getest. Het is veel effici\u00ebnter om dit op losse bacteri\u00ebn in suspensie te doen dan op volwassen biofilms, die minstens 5 dagen gekweekt moeten worden. In hoofdstuk 4 is onderzocht of de resultaten van deze combinaties op bacteri\u00ebn in suspensie voorspellend zijn voor hun effectiviteit tegen biofilms op metalen schijfjes. Uit de resultaten blijkt dat de verminderde effecten van bacteriofaag-antibioticacombinaties op biofilms goed kunnen worden voorspeld op basis van testen met bacteri\u00ebn in suspensie. Helaas worden de versterkte effecten van bacteriofaag-antibioticacombinaties in biofilms onderschat in testen met bacteri\u00ebn in suspensie. Desondanks kan deze testmethode worden gebruikt om ineffectieve combinaties uit te sluiten voordat de behandeling van (biofilm-gerelateerde) infecties met bacteriofagen en antibiotica wordt gestart. Tevens bleek dat dagelijkse toediening van bacteriofaag antibioticacombinaties gedurende twee of drie opeenvolgende dagen effectiever is dan een eenmalige behandeling van 24 uur.\n\nNaast bacteriofagen richt dit proefschrift zich ook op inductieverhitting van implantaten en antimicrobi\u00eble peptiden als alternatieve therapie\u00ebn. Inductieverhitting maakt gebruik van dezelfde technologie als inductiekookplaten, waarbij uitsluitend metalen objecten kunnen worden verhit. Op deze manier kunnen ge\u00efnfecteerde metalen implantaten contactloos en selectief worden opgewarmd door middel van een spoel die een wisselend magnetisch veld genereert. Het blootstellen van bacteri\u00ebn in biofilms aan hitte kan effectief zijn, aangezien hoge temperaturen belangrijke bacteri\u00eble processen en structuren kunnen beschadigen. Hitte die wordt gegenereerd met inductieverhitting kan lokaal worden toegepast en nauwkeurig worden gecontroleerd via de frequentie en de sterkte van het magnetisch veld, waardoor schade aan omliggende weefsels beperkt blijft.\n\nIn hoofdstuk 5 is aangetoond dat het zelfontwikkelde inductieverhittingsapparaat in staat is om binnen enkele minuten kleine metalen schijfjes te verhitten tot 80 \u00b0C. Verhitting van deze metalen met daarop biofilms resulteert in een sterke vermindering van levende bacteri\u00ebn, waaronder bacteri\u00ebn in \u2018slaaptoestand\u2019 die tijdelijk ongevoelig zijn voor antibiotica. Dit maakt inductieverhitting een veelbelovende strategie wanneer antibiotica onvoldoende effectief zijn. De combinatie van inductieverhitting met het antimicrobi\u00eble peptide SAAP-148 blijkt iets effectiever in het doden van deze bacteri\u00ebn dan beide behandelingen afzonderlijk. Hoewel met inductieverhitting specifiek het metalen implantaat wordt verhit, kan er warmte worden afgegeven aan de omliggende weefsels. Om weefselschade zoveel mogelijk te beperken, is het wenselijk om lagere temperaturen te gebruiken en inductieverhitting te combineren met andere behandelingen. In hoofdstuk 6 is aangetoond dat inductieverhitting bij lagere temperaturen, gecombineerd met opeenvolgende blootstelling aan antibiotica of SAAP-148, de effectiviteit tegen biofilms op metalen schijfjes vergroot. Dit was echter niet het geval bij inductieverhitting in combinatie met bacteriofaag ISP. Daarnaast kan inductieverhitting bacteri\u00ebn in biofilms gevoeliger maken voor antibiotica waar zij eerder resistent tegen waren.\n\nIn hoofdstuk 7 worden de bevindingen van dit proefschrift samengevat en bediscussieerd. De belangrijkste conclusie is dat een combinatie van behandelingen, tezamen met het operatief reinigen en verwijderen van het ge\u00efnfecteerde en dode weefsel, een veelbelovende toevoeging kan zijn om de behandeling van implantaat-gerelateerde infecties te verbeteren. Bovendien worden aanbevelingen gedaan voor toekomstig onderzoek. Zo zou meer aandacht moeten worden besteed aan bacteri\u00ebn die uit biofilms vrijkomen en aan bacteri\u00ebn die (tijdelijk) ongevoelig worden voor behandelingen. Toekomstige studies kunnen klinisch relevanter worden gemaakt door de omgeving van de gastheer beter na te bootsen, bijvoorbeeld door het toevoegen van afweercellen en lichaamsvloeistoffen aan bestaande infectiemodellen.","summary":"Metal biomaterials are widely used in surgery, particularly for joint replacements and bone fixations after trauma. The use of these materials is expected to increase in the future due to an aging society, obesity, and the global accessibility of these procedures. Although these medical aids undeniably contribute to pain reduction, improved mobility, and enhanced quality of life, infections occur in approximately 2\u201331% of patients following these surgeries. Infections around metal prosthetic joints and fracture-related metal materials are primarily caused by Staphylococcus bacteria, such as Staphylococcus aureus.\n\nCurrent treatment for acute infections consists of thoroughly cleaning the implant and surgically removing infected and necrotic tissue surrounding it, followed by at least 6 weeks of antibiotic treatment. This procedure, known as DAIR, is successful in about 70% of cases. When DAIR is ineffective, a decision may be made to repeat the procedure or replace the infected implant with a new one. Unfortunately, these latter operations are unsuccessful in approximately 14% of cases, requiring patients to use antibiotics long-term \u2014 sometimes for years \u2014 with all the associated side effects. Moreover, multiple operations are often necessary, and in rare cases, amputation is required to combat the infection. These additional interventions are accompanied by reduced mobility as well as increased pain and psychological distress, leading to a significant decrease in quality of life.\n\nImplant-associated infections are difficult to treat because bacteria such as Staphylococcus aureus can adhere to the metal surface. On the implant, they multiply, forming a protective layer of sugars, proteins, fats, and DNA around themselves (Chapter 1). This so-called biofilm forms a compact structure that is poorly penetrable for both antibiotics and immune system cells and factors. Additionally, biofilms are poorly visible and are often located in areas that are difficult for surgeons to reach, complicating the cleaning of the implant. Within biofilms, bacterial subpopulations arise with varying sensitivities to antibiotics. Some bacteria can enter a sort of 'hibernation,' making them temporarily less sensitive to antibiotics. Furthermore, bacteria \u2014 spontaneously or under the influence of antibiotics, for example \u2014 can be released from the biofilm and cause a new infection elsewhere in the body.\n\nThe failure of antibiotic treatment resulting from the aforementioned processes during biofilm formation contributes to the development of antibiotic resistance in bacteria. It is therefore necessary to develop alternative strategies to combat these infections and limit the development of resistance. The studies in this thesis focus on three alternatives to antibiotics and their combinations regarding biofilms: bacteriophages, viruses that kill bacteria (Chapters 2, 3, 4, and 6), heating the metal parts of the implant using induction (induction heating; Chapters 5 and 6), and antimicrobial peptides (Chapters 5 and 6).\n\nAntimicrobial peptides (AMPs) are small molecules that occur in nature (including in the human immune system) and are active against various pathogens. AMPs can destroy the bacterial cell wall, causing the bacterium to deflate like a balloon and die. Synthetic variants have been developed based on natural AMPs. One of these is SAAP-148, which is highly effective against Staphylococcus aureus, among others.\n\nBacteriophages are natural predators of bacteria and are highly specific to certain bacterial species and strains. Once these viruses recognize specific structures on the surface of the bacterium (cell wall), they inject their genetic material. Subsequently, the bacteriophages take over the bacterium's internal systems, causing it to produce new bacteriophages. In the final phase of this viral cycle, the bacterium bursts open, releasing all newly formed bacteriophages, and the bacterium dies. These offspring can then infect and kill other susceptible bacteria.\n\nCurrently, bacteriophage therapy is exclusively applied in the Western world as a last resort for difficult-to-treat infections. Partly due to this, little is still known about the effectiveness of phage therapy against bacteria in the various stages of the biofilm. To investigate this, the different phases of the biofilm were mimicked in the laboratory on small metal discs made of the same material as clinical implants. Chapter 2 describes research into the effects of bacteriophage ISP, which specifically targets Staphylococcus aureus. Although ISP is highly effective against bacteria in suspension, its effectiveness decreases as bacteria adhere to the metal surface and form a biofilm. Nevertheless, ISP is capable of penetrating the biofilm and interacting with the bacteria present. However, the quantity of bacteriophages in the biofilm is significantly lower than that to which the biofilm was initially exposed. Additionally, ISP is less effective against bacteria in a 'hibernation state,' as well as against bacteria spontaneously released from biofilms. For these bacteria, the insensitivity to ISP is only temporary.\n\nThe research described in Chapter 3 aimed to identify the survival mechanisms of bacteria that are spontaneously released from biofilms and exposed to bacteriophages. The results show that a subpopulation of these bacteria is temporarily insensitive to the bacteriophages ISP and Sb-1 at high doses. In the study, all proteins of sensitive and insensitive bacteria were compared. Based on this, two possible survival mechanisms were identified: a reinforced cell wall and a reduced metabolism. Despite the reinforced cell wall, bacteriophages can still inject their genetic material. It is possible that phages are unable to open the bacterial cell wall in the final stage. A reduced metabolism possibly also ensures that fewer phages are produced, which likewise contributes to the survival of the bacteria as a group.\n\nIn practice, bacteriophage therapy is often combined with antibiotics, with the choice of these combinations depending on the sensitivity of the bacterium to both treatments. Phages and antibiotics can enhance each other's effect or weaken it, making the selection of the right combination crucial. For this, the effectiveness of phage-antibiotic combinations must be tested. It is much more efficient to do this on loose bacteria in suspension than on mature biofilms, which must be grown for at least 5 days. Chapter 4 investigates whether the results of these combinations on bacteria in suspension are predictive of their effectiveness against biofilms on metal discs. The results show that the reduced effects of phage-antibiotic combinations on biofilms can be well predicted based on tests with bacteria in suspension. Unfortunately, the enhanced effects of these combinations in biofilms are underestimated in tests with bacteria in suspension. Nevertheless, this testing method can be used to exclude ineffective combinations before starting treatment. Furthermore, daily administration of phage-antibiotic combinations over two or three consecutive days was found to be more effective than a single 24-hour treatment.\n\nIn addition to bacteriophages, this thesis also focuses on induction heating of implants and antimicrobial peptides as alternative therapies. Induction heating uses the same technology as induction cooktops, where only metal objects can be heated. In this way, infected metal implants can be warmed contactlessly and selectively by means of a coil that generates an alternating magnetic field. Exposing bacteria in biofilms to heat can be effective, as high temperatures can damage important bacterial processes and structures. Heat generated with induction heating can be applied locally and accurately controlled via the frequency and strength of the magnetic field, limiting damage to surrounding tissues.\n\nChapter 5 demonstrates that the self-developed induction heating device is capable of heating small metal discs to 80 \u00b0C within a few minutes. Heating these metals with biofilms results in a strong reduction of live bacteria, including bacteria in a 'hibernation state' that are temporarily insensitive to antibiotics. This makes induction heating a promising strategy when antibiotics are insufficiently effective. The combination of induction heating with the antimicrobial peptide SAAP-148 appears slightly more effective in killing these bacteria than both treatments separately. Although induction heating specifically heats the metal implant, heat can be transferred to surrounding tissues. To minimize tissue damage as much as possible, it is desirable to use lower temperatures and combine induction heating with other treatments. Chapter 6 shows that induction heating at lower temperatures, combined with sequential exposure to antibiotics or SAAP-148, increases effectiveness against biofilms. However, this was not the case for induction heating combined with bacteriophage ISP. Additionally, induction heating can make bacteria in biofilms more sensitive to antibiotics to which they were previously resistant.\n\nChapter 7 summarizes and discusses the findings of this thesis. The main conclusion is that a combination of treatments, together with surgical cleaning and removal of infected and necrotic tissue, is a promising addition to improve the treatment of implant-related infections. Furthermore, recommendations are made for future research. More attention should be paid to bacteria released from biofilms and to bacteria that (temporarily) become insensitive to treatments.
