De review bestaat uit 15 studies (8 gerandomiseerde en 7 observationele) met in totaal 984 pati\u00ebnten. De resultaten toonden dan dat de intermitterende levosimendan infusies geassocieerd zijn met verbeterde functionele klasse volgens de New York Heart Association (NYHA), toegenomen linkerventrikel ejectiefractie en zorgt voor een afname van de B-type natriuretische peptide (BNP). Het algehele overlijdensrisico verschilde niet significant, maar overlijden gerelateerd aan cardiovasculair problematiek kwam minder voor in de groep met levosimendan infusies. Tevens werd er een verbetering van de kwaliteit van leven gevonden in de levosimendan groep. De bijwerkingen waren vergelijkbaar tussen de levosimendan- en placebogroep.\n\nHoofdstuk 3 onderzocht de associatie tussen infecties en trombo-embolische complicaties, met name beroertes, in pati\u00ebnten met een LVAD. De studie omvatte data vanuit de European Registry for Patients Assisted with Mechanical Circulatory Support, met in totaal 3282 LVAD pati\u00ebnten. De studie toonde aan dat het optreden van een infectie het risico op een beroerte verhoogd, met een hazard ratio van 1.90. Verdere multivariate analyse bevestigde deze associatie, met een hazard ratio van 1.99. Het verdelen van de infecties aan de hand van de origine in verschillende categorie\u00ebn toonde aan dat VAD-specifieke en VAD-gerelateerde infecties geassocieerd waren met meer beroertes.\n\nHoofdstuk 4 beschrijft de relatie tussen de hoek van de LVAD inflow-canule en herhaaldelijke lage flow alarmen. De studie bevatte 48 pati\u00ebnten met een LVAD HMP. Er werd een gestandaardiseerd en reproductief protocol gemaakt om de positie van de inflow-canule te meten met behulp van contrast CT-scans. De studie toonde een significante toename van lage flow alarmen wanneer de septaal-laterale hoek 28\u00b0 of meer was. De lage flow alarmen namen in de loop van de tijd toe naarmate de hoek zich uitbreidde naar het septaal-laterale vlak, dit had een constante toename van 0,031 lage flow alarmen per maand per graad (P = 0,048). Daarentegen vertoonden de anterieur-posterieur en maximale angulatie van de inflow-canule geen significante verschillen. Dit benadrukt het belang van het juist positioneren van de inflow-canule om herhaaldelijke lage flow alarmen te voorkomen, met het risico op verminderde levenskwaliteit en andere morbiditeit.\n\nHoofdstuk 5 onderzocht de incidentie van mechanische storingen bij het gebruik van de HeartMate II en HeartMate 3. De studie omvatte 163 pati\u00ebnten, waarvan 39% met een HeartMate II en 61% met een HeartMate 3. De mediane LVAD-ondersteuning tijd was, respectievelijk, 24,6 en 21,1 maanden. Kritische en potentieel kritische mechanische storingen kwamen significant minder voor bij pati\u00ebnten met een HeartMate 3, met een hazard ratio van 0,37. Er waren eveneens minder pomp- of uitstroom graft vervangingen nodig bij pati\u00ebnten met een HeartMate 3. Hoewel systeemcontroller defecten minder vaak voorkwamen bij de HeartMate 3, waren batterijklem defecten vaker voorkomend. De studie concludeerde dat de HeartMate 3 minder kritische storingen vertoonde en benadrukte het belang van voortdurende technische controles tijdens LVAD-ondersteuning.\n\nHoofdstuk 6 onderzocht de impact van een pectus excavatum op de vroege en late resultaten, met name rechterventrikel falen, na LVAD-implantatie. De studie bevatte 80 pati\u00ebnten en deze werden ingedeeld in twee groepen op basis van borstkasafmetingen volgens de Haller-Index: 28 met een normale borstkas en 52 met een pectus excavatum. Vroeg rechterventrikel falen, acuut nierinsuffici\u00ebntie en algehele mortaliteit verschilden niet tussen beide groepen. Late heropnames voor rechterventrikel falen kwamen echter wel vaker voor bij pati\u00ebnten met pectus excavatum. Het begin van laat rechterventrikel falen trad ongeveer 18 maanden na implantatie op en nam daarna geleidelijk toe in de algehele studiepopulatie. De studie laat zien dat pectus excavatum vaak wordt gezien bij LVAD-pati\u00ebnten en dat het geassocieerd is met een verhoogd risico op laat rechterventrikel falen met de daarbij behorende heropnames.\n\nHoofdstuk 7 presenteert het effect van de positie van de uitstroomgraft op trombo-embolische complicaties en aortaklepinsuffici\u00ebntie bij pati\u00ebnten met een LVAD. De studie omvatte CT-scans met contrast van 59 pati\u00ebnten. Driedimensionale reconstructies werden gemaakt en gebruikt om de horizontale en verticale hoeken, evenals de relatieve afstand van de uitstroomgraft tussen de aortaklep en de arteria branchiocephalica te berekenen. Een verticale hoek \u2265 107\u00b0 was significant geassocieerd met een verhoogd risico op beroertes en gastro-intestinale bloedingen gedurende een mediane follow-up van 25 maanden. Er werden geen significante associaties gevonden tussen de verticale hoek en aortaklepinsuffici\u00ebntie of de overleving.\n\nHoofdstuk 8 onderzocht de externe compressie van de uitstroomgraft bij pati\u00ebnten met een HeartMate 3 LVAD. De studie is een multicenteranalyse van 2108 pati\u00ebnten en is gedaan tussen november 2014 en april 2021 welke werd uitgevoerd in 17 centra in 8 landen. Externe obstructie van de uitstroomgraft werd gedefinieerd als een obstructie van meer dan 25% van de dwarsdoorsnede van de uitstroomgraft. De prevalentie van externe obstructie van de uitstroomgraft was 3,0%, en de incidentie nam toe in de loop van de tijd: 0,6%, 2,8%, 4,0%, 5,2% en 9,1% na respectievelijk 1, 2, 3, 4 en 5 jaar. In totaal werden 62 pati\u00ebnten gediagnosticeerd met externe obstructie van de uitstroomgraft en er werden verschillende interventies toegepast; observatie, chirurgische revisie, percutane stentplaatsing en harttransplantatie. De mortaliteit bij therapeutische interventie was 17%. Hoewel het relatief weinig voorkomt, kan externe obstructie van de uitstroomgraft voorkomen worden bij LVAD-ondersteunde pati\u00ebnten. Dit benadrukt het belang van vroegtijdige detectie en alertheid van de artsen.\n\nHoofdstuk 9 beschrijft de associatie tussen anatomische variaties in de aftakkingen van de aortaboog en complicaties, met name het risico op beroertes, bij pati\u00ebnten met een LVAD. De studie omvatte 101 pati\u00ebnten, waarbij CT-scans werden gebruikt om de variaties in de aftakkingen van de aortaboog in de zeven typen te classificeren. De studie vond geen significante verschillen in het percentage vroegtijdige exploratie vanwege bloeding na LVAD-implantatie, beroertes en overlijden gedurende de follow-up. De studie suggereert dat, hoewel er geen verschillen waren in nadelige gebeurtenissen, kennis van variaties in de aortaboog zinvol zou kunnen zijn tijdens de operatie.\n\nHoofdstuk 10 presenteert de klinische impact en het lange termijn beloop van tricuspidalisklep insuffici\u00ebntie en dynamische aard hiervan bij 572 pati\u00ebnten na harttransplantatie. Ongeveer 32% van de pati\u00ebnten hadden direct na de operatie matige tot ernstige tricuspidalisklep insuffici\u00ebntie, dit daalde tot 11% na 5 jaar en 9% na 10 jaar na transplantatie. Overlevingspercentages na 1, 5, 10 en 20 jaar waren respectievelijk 97%, 88%, 66% en 23%. Matige tot ernstige tricuspidalisklep insuffici\u00ebntie tijdens de follow-up was geassocieerd met een verhoogde mortaliteit. Het verloop van tricuspidalisklep insuffici\u00ebntie vertoonde een positieve correlatie met het verloop van creatinine, wat wijst op een associatie met een slechtere nierfunctie. De studie suggereert dat tricuspidalisklep insuffici\u00ebntie significant geassocieerd is met een hogere mortaliteit en een slechtere nierfunctie; de kans op tricuspidalisklep insuffici\u00ebntie is het hoogst direct na harttransplantatie en neemt in de loop van de tijd af.\n\nHoofdstuk 11 is een systematische review waarbij de biatriale en bicavale harttransplantatie techniek wordt vergeleken in de vroege en late fase. In totaal werden er 36 publicaties (bestaande uit 3555 biatriale- en 3208 bicavale harttransplantaties) ge\u00efncludeerd. Vroege resultaten, waaronder mortaliteit, tricuspidalisklep insuffici\u00ebntie, mitralisklep insuffici\u00ebntie en permanente pacemaker implantatie, kwamen minder vaak voor wanneer er gebruik werd gemaakt van de bicavale techniek. Bovendien was de lange termijn overleving en vrijheid van tricuspidalisklep insuffici\u00ebntie superieur bij de bicavale techniek.\n\nHoofdstuk 12 bespreekt de resultaten van de verschillende studies, klinische implicaties, geeft antwoord op de onderzoeksvragen en stelt suggesties voor vervolgonderzoek voor.","summary":"Chapter 1 gives a general introduction to this thesis and describes the aims and brief outline of this thesis.\n\nChapter 2 aimed to assess the safety and efficacy of intermittent levosimendan infusions in ambulatory patients with end-stage heart failure in a systematic review. The review included 15 studies (8 randomized and 7 observational) encompassing 984 patients. Levosimendan infusions were linked to improved New York Heart Association (NYHA) functional class, increased left ventricular ejection fraction, and led to a decrease of the B-type natriuretic peptide (BNP) levels. All-cause death did not differ significantly, but cardiovascular death was lower in the group with levosimendan infusions. An improvement in the health-related quality of life was seen, and adverse events were similar between levosimendan and placebo groups.\n\nChapter 3 investigated the association between infections and thromboembolic events, particularly cerebrovascular accidents during LVAD support. This study included data from the European Registry for Patients Assisted with Mechanical Circulatory Support, with 3282 LVAD patients. During the study, 1262 patients experienced infections, and 457 experienced a cerebrovascular accident. The contemporary analysis showed that the occurrence of an infection increased the risk of cerebrovascular accidents, with a hazard ratio of 1.90. Multivariable analysis confirmed this association, with a hazard ratio of 1.99. Categorization of infections showed that VAD-specific and VAD-related infections were associated with cerebrovascular accidents. This study emphasizes the importance of rigorous anticoagulation management and appropriate antibiotic treatment during infections in LVAD-supported patients.\n\nChapter 4 describes the relationship between the angular position of the LVAD inflow cannula and relapsing low-flow alarms. The study included 48 LVAD patients. A standardized protocol was made to measure the inflow cannula position by using contrast-enhanced computed tomography scans. The study showed a significant increase in low flow alarms when the septal\u2013lateral angulation was 28\u00b0 or more. The low-flow alarms increased over time as the angulation expanded towards the septal\u2013lateral plane, with a constant rate function indicating a 0.031 increase in low-flow alarms per month per degree of angulation (P = 0.048). In contrast, anterior\u2013posterior and maximal inflow cannula angulation did not show significant differences. This emphasizes the importance of the LVAD inflow cannula angular position to prevent relapsing low-flow alarms with the risk of diminished quality of life and morbidity.\n\nChapter 5 investigated the incidence of mechanical device malfunction in HeartMate II and HeartMate 3 LVADs. The analysis included 163 patients, with 39% receiving HeartMate II and 61% receiving HeartMate 3, and median support times of 24.6 and 21.1 months. Major and potential major mechanical device malfunction occurred significantly less in HeartMate 3 patients, with a hazard ratio of 0.37. HeartMate 3 patients also had a significantly lower hazard of requiring pump or outflow graft exchange. Although system controller defects occurred less frequently in HeartMate 3, battery-clip defects were more common. The study concludes that HeartMate 3 exhibited fewer major device malfunctions and highlighted the importance of ongoing technical assessments during long-term LVAD support.\n\nChapter 6 explores the impact of pectus excavatum on early and late outcomes, particularly right ventricular failure, following LVAD implantation. The study included 80 patients and was categorized into two groups based on chest wall dimensions: 28 with a normal chest wall and 52 with pectus excavatum. Early RVF, acute kidney injury, and overall mortality did not differ between groups. However, late recurrent readmissions for RVF were more common in patients with pectus excavatum. The onset of late RVF occurred around 18 months post-implantation and increased thereafter in the overall cohort. The study suggests that pectus excavatum is frequently observed in LVAD patients and is associated with increased late RVF and readmissions.\n\nChapter 7 presents the potential impact of the angular position of the outflow graft on thromboembolic events and aortic valve regurgitation in patients with LVAD. The analysis involved contrast-computed tomography data from 59 patients. Three-dimensional reconstructions were used to calculate the horizontal and vertical angles, as well as the relative distance of the outflow graft between the aortic valve and the brachiocephalic artery. A vertical angle \u2265 107\u00b0 was significantly associated with an increased risk of cerebrovascular accidents and gastrointestinal bleeding during a median 25-month follow-up. No significant associations were found between the vertical angle and aortic valve regurgitation or survival.\n\nChapter 8 investigated external compression of the outflow graft obstructing patients with a HeartMate 3 LVAD. This study is a multi-center analysis of 2108 patients between November 2014 and April 2021 and was conducted across 17 cardiac centers in 8 countries. External outflow graft obstruction was defined as obstruction >25% in the cross-sectional area in imaging. The prevalence of external outflow graft obstruction was 3.0%, and the incidence increased over time: 0.6%, 2.8%, 4.0%, 5.2%, and 9.1% at 1, 2, 3, 4, and 5 years of support. In total, 62 patients were diagnosed with external outflow graft obstruction and interventions included observation, surgical revision, percutaneous stent implantation, heart transplant, and death. Mortality with therapeutic intervention was 17.0%. While uncommon, external outflow graft obstruction may occur in LVAD-supported patients, highlighting the importance of early detection and remaining vigilant by clinicians.\n\nChapter 9 describes the association between anatomical variations of the aortic arch branching and adverse events, particularly the risk of cerebrovascular accidents, in patients with LVAD. The analysis involved 101 patients and computed tomography scans were used to categorize aortic arch branching variations into seven types. The study found no significant differences in the rate of early re-exploration due to bleeding after LVAD implantation, cerebrovascular accidents, and mortality during follow-up. The study suggests that while there were no differences in adverse events, knowledge of aortic arch variations could be meaningful during cardiac surgery.\n\nChapter 10 presents the clinical impact and long-term course of tricuspid regurgitation examined, considering its dynamic nature in 572 patients after heart transplantation. About 32% of patients had moderate-to-severe TR immediately after surgery, but this declined to 11% at 5 years and 9% at 10 years after transplantation. Preimplant mechanical support was associated with less tricuspid regurgitation during follow-up, while concurrent left ventricular dysfunction was linked to more tricuspid regurgitation. Survival rates at 1, 5, 10, and 20 years were 97%, 88%, 66%, and 23%, respectively. Moderate-to-severe TR during follow-up was associated with an increased mortality. The course of TR was positively correlated with the course of creatinine, indicating an association with worse renal function. The study suggests that tricuspid regurgitation is significantly associated with higher mortality and worse renal function, the probability of tricuspid regurgitation is highest immediately after heart transplantation and decreases over time.\n\nChapter 11 provides a systematic comparison of biatrial and bicaval heart transplantation in the early and late phases. In total, 36 publications (3555 biatrial and 3208 bicaval heart transplantations) were included. Early outcomes, including mortality, tricuspid regurgitation, mitral regurgitation, and permanent pacemaker implantation, favored the bicaval technique.
